Rubber band ligation for internal hemorrhoids?

Hello Doctor, I have already undergone two hemorrhoid ligation surgeries, and my condition has indeed improved, but I still experience prolapse of the hemorrhoids.
I would like to ask two questions:
1.
After the ligation surgery, will the ligated hemorrhoids prolapse during bowel movements the next day? I read in previous responses that two hours post-surgery is sufficient to cut off the blood supply, so I would like to know if, after 24 hours post-ligation, the hemorrhoids can still prolapse due to congestion.
2.
Additionally, I would like to inquire about the current number of procedures covered by health insurance.
I found online that some say it is four times, but when I asked my doctor in the outpatient clinic, they mentioned it is three times.
Thank you for your response!

1.
After hemorrhoid ligation, there is still a possibility of prolapse, which may be due to the protrusion of hemorrhoidal tissue that was not ligated or because the ligated hemorrhoids were too large to be completely ligated in one session.

2.
The National Health Insurance covers the procedure up to four times a year.
Previously, there was a requirement for a minimum interval of two weeks between procedures, but this is no longer restricted.
However, it is generally not recommended to have too short an interval between two ligation procedures.

After undergoing two internal hemorrhoid ligation surgeries, it's understandable that you have concerns about the healing process and the potential for complications. Let's address your questions one by one.

1. Will the ligated hemorrhoids prolapse after surgery?
After a ligation procedure, the goal is to cut off the blood supply to the hemorrhoids, which should lead to their shrinkage and eventual disappearance. Typically, ligation is effective within a few hours, and by the next day, the blood supply to the treated hemorrhoids should be significantly reduced. However, it is still possible for the hemorrhoids to prolapse or protrude, especially if there is increased pressure in the rectal area during bowel movements. Factors such as straining, constipation, or even the natural healing process can contribute to this. If you experience prolapse, it may not necessarily indicate a failure of the procedure but could be related to the healing process or the presence of other hemorrhoids that were not treated. It's essential to follow your doctor's advice regarding post-operative care, including dietary recommendations to prevent constipation and straining.

2. What is the coverage for hemorrhoid ligation under health insurance?
The number of covered ligation procedures can vary based on the specific health insurance policy and the regulations in your region. Generally, in many healthcare systems, there may be a limit on the number of times a procedure can be performed within a certain timeframe. You mentioned that you found conflicting information online regarding the number of covered procedures—some sources stating four times and others three. The best course of action is to consult directly with your healthcare provider or the billing department of your hospital or clinic. They can provide you with the most accurate and up-to-date information regarding your insurance coverage and any limitations that may apply.

In addition to addressing your specific questions, it's important to consider some general post-operative care tips for hemorrhoid ligation:
- Dietary Adjustments: Focus on a high-fiber diet to promote soft bowel movements. Foods rich in fiber, such as fruits, vegetables, whole grains, and legumes, can help prevent constipation and reduce straining during bowel movements.

- Hydration: Drink plenty of fluids to help keep your stool soft and prevent dehydration, which can lead to constipation.

- Avoid Straining: When using the bathroom, try to relax and avoid straining, as this can put pressure on the treated area and potentially lead to complications.

- Follow-Up Appointments: Keep all follow-up appointments with your healthcare provider to monitor your recovery and address any concerns you may have.

- Pain Management: If you experience discomfort or pain after the procedure, consult your doctor about appropriate pain management strategies, which may include over-the-counter pain relievers or topical treatments.

- Watch for Complications: Be vigilant for signs of complications, such as excessive bleeding, severe pain, or signs of infection (e.g., fever, increased swelling, or discharge). If you notice any concerning symptoms, contact your healthcare provider promptly.

In conclusion, while it's common to have concerns after surgical procedures, maintaining open communication with your healthcare provider and following their recommendations will help ensure a smoother recovery. If you have further questions or experience any unusual symptoms, don't hesitate to reach out to your doctor for guidance.
