Fever of unknown origin
Hello Doctor: I have been experiencing unexplained fever since May 20th.
I have seen both immunology and rheumatology specialists, as well as infectious disease specialists, and have undergone all necessary blood and urine tests, all of which came back normal.
I also had a bone marrow biopsy, which was also normal.
However, I still experience fevers reaching 38 degrees Celsius.
The fever subsides with antipyretics during the day, but it tends to recur at night.
Prior to the fever, I feel weakness and soreness in my knees.
I am very troubled and would like to know which specialist I should consult next to resolve my fever issue.

Dr. Lin Wufu reply Internal Medicine
Hello, undiagnosed fever is inherently a challenging diagnostic issue, with some individuals requiring follow-up for one to two years before the underlying cause gradually becomes apparent.
There are also many cases where the fever resolves on its own over time.
It is recommended to continue monitoring in an infectious disease outpatient clinic, where the physician will arrange relevant tests based on clinical changes.

Unexplained fevers can be particularly frustrating and challenging to diagnose, especially when initial tests for common infectious and immunological causes return normal results. Your experience of having persistent fevers since May, despite thorough evaluations by specialists in immunology and infectious diseases, suggests that we may need to consider a broader range of potential causes.
Firstly, it’s important to understand that unexplained fevers can arise from a variety of conditions beyond infections and immune system disorders. These can include:
1. Autoimmune Disorders: While you have already consulted with specialists, some autoimmune conditions can present with fever as a symptom, even when other markers appear normal. Conditions like Still's disease or systemic lupus erythematosus (SLE) can sometimes cause intermittent fevers.
2. Malignancies: Certain cancers, particularly lymphomas and leukemias, can present with unexplained fevers. Although you have undergone a bone marrow biopsy, it may be worth discussing further imaging studies or consultations with an oncologist if your symptoms persist.
3. Endocrine Disorders: Conditions such as hyperthyroidism or adrenal insufficiency can lead to unexplained fevers. A thorough evaluation of your endocrine system, including thyroid function tests and adrenal function tests, may be warranted.
4. Chronic Inflammatory Conditions: Conditions like sarcoidosis or vasculitis can cause systemic inflammation that results in fever. These conditions may not always show up in standard blood tests, so further specialized testing might be necessary.
5. Infectious Diseases: While you have been evaluated for common infections, there are less common infections (like certain viral infections, tuberculosis, or even atypical infections) that may require specific tests. A consultation with an infectious disease specialist may provide insights into further testing options.
6. Environmental Factors: Sometimes, exposure to certain environmental factors, such as allergens or toxins, can lead to systemic reactions that manifest as fever. A detailed history of your environment and any recent changes could be beneficial.
Given your symptoms of knee discomfort prior to fever episodes, it may also be worth considering rheumatological evaluations. Conditions like reactive arthritis or other forms of inflammatory arthritis can present with joint pain and fever.
In terms of next steps, I recommend the following:
- Consult a Rheumatologist: Given your symptoms and the history of joint discomfort, a rheumatologist may be able to provide further insights into potential autoimmune or inflammatory conditions that could be causing your fevers.
- Further Imaging Studies: If not already done, imaging studies such as a CT scan or MRI may help identify any underlying issues that blood tests cannot.
- Holistic Review of Symptoms: Keep a detailed diary of your symptoms, including the timing of fevers, associated symptoms, and any potential triggers. This information can be invaluable for your healthcare provider in identifying patterns that may point to a specific diagnosis.
- Consider a Multidisciplinary Approach: Sometimes, a team approach involving multiple specialties can yield better results in diagnosing complex cases. Discuss with your primary care physician the possibility of involving other specialists.
In conclusion, while unexplained fevers can be daunting, a systematic approach involving further specialized evaluations and a thorough review of your symptoms can help uncover the underlying cause. It’s essential to maintain open communication with your healthcare providers and advocate for further investigations as needed.
