Does frequent ligation of internal hemorrhoids affect anal function?

Hello Doctor, over the past 2 to 3 years, I have frequently experienced anal swelling and have undergone internal hemorrhoid ligation surgery about ten times.
Six months ago, I had a hemorrhoidal excision with a harmonic scalpel (HET), but the internal hemorrhoids have recurred, and I am starting to feel anal swelling again.
I would like to ask if such frequent internal hemorrhoid ligation could affect anal function, such as causing anal stenosis? Thank you.

Niming, 30~39 year old female. Ask Date: 2022/08/10

Dr. Zhang Shuhao reply Surgery

Frequent rubber band ligation for hemorrhoids does not affect anal function, but the effectiveness of the ligation may diminish over time.
Symptom relief is mostly achieved through medication and warm sitz baths.
If episodes occur frequently and impact quality of life, surgical removal is still recommended.

Frequent hemorrhoid banding, also known as rubber band ligation, is a common procedure used to treat hemorrhoids, particularly internal hemorrhoids. While this procedure can be effective in alleviating symptoms such as bleeding and discomfort, there are concerns regarding its potential impact on anal function, especially when performed repeatedly.

Firstly, it is essential to understand how hemorrhoid banding works. The procedure involves placing a rubber band around the base of the hemorrhoid, cutting off its blood supply. Over time, the hemorrhoid shrinks and eventually falls off. This method is generally considered safe and effective for treating hemorrhoids, but like any medical procedure, it carries risks and potential complications.

One of the primary concerns with frequent banding is the possibility of anal stenosis, which is a narrowing of the anal canal. This condition can result from scar tissue formation due to repeated trauma from the banding procedure. Symptoms of anal stenosis may include difficulty with bowel movements, pain during defecation, and a feeling of incomplete evacuation. In severe cases, it can lead to significant discomfort and require further medical intervention.

Additionally, frequent banding may also affect the anal sphincter function. The anal sphincters are muscles that control the passage of stool and gas. If the procedure leads to excessive scarring or changes in the tissue structure, it could potentially weaken the sphincter muscles, resulting in issues such as fecal incontinence or urgency.

Moreover, repeated procedures can lead to changes in the normal anatomy of the anal region. The risk of complications increases with the number of procedures performed. For instance, if the hemorrhoids recur frequently, it may indicate an underlying issue that needs to be addressed, such as dietary factors, chronic constipation, or other gastrointestinal conditions.

In your case, having undergone approximately ten banding procedures over the past two to three years, it is crucial to monitor any changes in anal function. If you are experiencing persistent symptoms such as anal swelling or discomfort, it may be beneficial to consult with a colorectal specialist. They can assess your condition and determine whether further intervention is necessary or if alternative treatments might be more appropriate.

In conclusion, while hemorrhoid banding is a generally safe procedure, frequent applications can potentially affect anal function, leading to complications such as anal stenosis or sphincter dysfunction. It is essential to have open communication with your healthcare provider about your symptoms and treatment options. They can help you weigh the benefits and risks of continued banding versus exploring other therapeutic avenues, such as dietary modifications, lifestyle changes, or even surgical options if warranted. Regular follow-ups and assessments will be key in managing your condition effectively and maintaining anal function.
