Unexplained fever and diarrhea?

I have been experiencing severe diarrhea, regardless of whether I eat or not.
My body feels constantly hot, I don't sweat, I can't sleep, and I have pain all over.
Both colonoscopy and gastroscopy showed no issues.
I have taken medication for irritable bowel syndrome, but it hasn't improved my diarrhea.
I have tried anti-diarrheal medications, but they haven't stopped it either.
I feel weak and get goosebumps, but my weight hasn't decreased at all.
I am very thirsty, but after drinking, I quickly need to use the restroom.
Could you please recommend which specialty I should consult? Thank you, doctor.

It is recommended to consult or refer to the original gastroenterologist.

Based on your symptoms of unexplained fever, persistent diarrhea, and overall discomfort, it is essential to approach this situation with a comprehensive understanding of potential underlying causes. Given that you have already undergone both colonoscopy and gastroscopy without any significant findings, it may be time to consider other specialists and diagnostic avenues.

Firstly, your symptoms suggest a possible gastrointestinal infection, which could be viral, bacterial, or parasitic in nature. The fact that you are experiencing fever alongside diarrhea indicates that your body is likely fighting off an infection. It is also concerning that you have not found relief with antidiarrheal medications, which could suggest that the underlying issue is not simply a matter of gastrointestinal upset but may involve a more complex condition.

Given your current situation, I would recommend consulting with an Infectious Disease Specialist. This specialist can help identify if there is a specific infectious agent causing your symptoms. They may suggest further testing, such as stool cultures, PCR tests for specific pathogens, or even blood tests to check for markers of infection or inflammation.
Additionally, considering your symptoms of extreme thirst and frequent urination, it may be prudent to evaluate for conditions such as diabetes mellitus or other metabolic disorders. A Endocrinologist could be consulted to rule out these conditions, especially if you have a family history of metabolic issues or if you have experienced significant changes in your weight or energy levels.

Moreover, if your symptoms persist, a Gastroenterologist may still be beneficial, particularly if there are concerns about conditions like inflammatory bowel disease (IBD) or irritable bowel syndrome (IBS), which may not always show clear results on standard imaging or endoscopic procedures. They can provide further insights into dietary management and potential treatments that may alleviate your symptoms.

In the meantime, it is crucial to stay hydrated, especially since diarrhea can lead to dehydration. Drinking clear fluids, oral rehydration solutions, or electrolyte drinks can help maintain your hydration levels. Avoiding dairy products, caffeine, and high-fat foods may also be beneficial until your symptoms improve.

Lastly, if your fever persists or worsens, or if you experience additional symptoms such as severe abdominal pain, blood in your stool, or signs of dehydration (like dizziness, dry mouth, or decreased urination), it is vital to seek immediate medical attention. These could be signs of a more serious condition that requires prompt intervention.

In summary, I recommend consulting with an Infectious Disease Specialist to explore potential infections, while also considering an Endocrinologist for metabolic concerns. A Gastroenterologist may still be relevant if symptoms persist. Your health is paramount, and addressing these symptoms with the right specialists will help you find relief and clarity regarding your condition.
