Shortness of breath?
Hello doctor, recently I had an incident where I pushed a cart with a colleague to another floor.
It was a very short distance, but after walking that distance, I experienced difficulty breathing and felt the need to take deep breaths, but the more I tried, the less air I could get.
I started to break out in cold sweats, felt weakness in my limbs, and became dizzy.
Eventually, I was unable to stand up; standing made me feel very uncomfortable and I had a slight urge to vomit.
It took about 20-25 minutes before I could finally stand up.
This was the first time I felt something was seriously wrong, especially the inability to breathe properly.
Then yesterday, I forcefully bumped my shoulder (above the collarbone) against a table leg, which was very painful, and shortly after that, I experienced difficulty breathing again.
I had to remove my mask and lean against the wall to take deep breaths, and I was gasping for air.
Recently, in addition to the shortness of breath, I have also been experiencing a sensation in my ears where I hear an echo of my own voice, which I’m not sure if it qualifies as tinnitus.
It feels like my voice is amplified in my head, similar to the sensation you get when you’re on an airplane.
Swallowing or pinching my nose and blowing does not alleviate this condition, and it often lasts for an entire day.
Could you please advise me on which specialty I should consult and what might be causing these symptoms?

More Info
Experiencing shortness of breath can be alarming, especially when accompanied by symptoms such as sweating, dizziness, and a feeling of impending doom. Based on your description, it is crucial to understand the potential causes and when to seek medical attention.
Understanding Shortness of Breath
Shortness of breath, or dyspnea, can arise from various conditions affecting the respiratory or cardiovascular systems. In your case, the sudden onset of breathlessness after minimal exertion, coupled with symptoms like cold sweats, weakness, and dizziness, suggests that your body may be struggling to get enough oxygen. This could be due to several factors, including but not limited to:
1. Respiratory Issues: Conditions such as asthma, chronic obstructive pulmonary disease (COPD), or even an acute respiratory infection can lead to difficulty in breathing. If you have a history of respiratory problems, this could be a contributing factor.
2. Cardiovascular Problems: Heart-related issues, such as arrhythmias or heart failure, can manifest as shortness of breath, especially during physical activity. The feeling of your heart racing or pounding could indicate a cardiac event that needs immediate attention.
3. Anxiety or Panic Attacks: Sometimes, psychological factors can lead to physical symptoms. Panic attacks can cause hyperventilation, leading to feelings of breathlessness, dizziness, and sweating.
4. Musculoskeletal Injuries: The incident where you hit your shoulder could have caused pain that affects your breathing pattern. Pain can lead to shallow breathing, which may exacerbate feelings of breathlessness.
5. Ear and Sinus Issues: The sensation of echoing in your ears could be related to pressure changes, possibly from sinus congestion or eustachian tube dysfunction, which can also affect your breathing.
When to Seek Medical Attention
Given the severity of your symptoms, especially the inability to breathe properly, it is advisable to seek medical attention promptly. Here are some guidelines on when to go to the doctor:
- Severe Shortness of Breath: If you experience significant difficulty breathing, especially if it feels like you cannot get enough air, this is a medical emergency.
- Chest Pain or Discomfort: If you have chest pain, pressure, or discomfort along with shortness of breath, seek immediate medical help as this could indicate a heart attack or other serious conditions.
- Persistent Symptoms: If your symptoms persist or worsen, such as ongoing dizziness, weakness, or feeling faint, it is essential to consult a healthcare provider.
- Acute Changes: Any sudden changes in your health status, such as the symptoms you described after pushing the cart or hitting your shoulder, warrant a medical evaluation.
Which Specialist to See
For your symptoms, starting with a visit to a primary care physician is a good idea. They can perform an initial evaluation and may refer you to specialists such as:
- Pulmonologist: If your symptoms are suspected to be related to lung or respiratory issues.
- Cardiologist: If there are concerns regarding your heart function.
- Ear, Nose, and Throat (ENT) Specialist: If your ear symptoms persist and are affecting your breathing.
Conclusion
In summary, your symptoms of shortness of breath, especially after minimal exertion, along with dizziness and sweating, are concerning and should not be ignored. It is essential to seek medical attention to rule out serious conditions and receive appropriate care. Early intervention can be crucial in managing any underlying health issues effectively. Remember, it is always better to err on the side of caution when it comes to your health.
