Arrhythmia
Hello Dr.
Huang,
A year ago, I was diagnosed with premature atrial contractions and mild mitral and tricuspid regurgitation, which are causing arrhythmia issues (I experience palpitations mostly at night while sleeping, and less frequently during the day).
I have been learning to play the jazz drums, but classes were suspended for a few months due to the pandemic.
During that time, my palpitations seemed to improve (occurring only a few times), but I am unsure if this was related to reduced academic pressure after graduation or the cessation of drumming (I have heard that high decibel noise can affect the heart).
Recently, I started playing the drums again, and I have begun to experience palpitations before bed.
Sometimes, when I sit or lie down and then stand up, I feel dizzy.
I would like to ask the doctor:
1.
Why do the symptoms of palpitations mostly occur at night while sleeping?
2.
Can playing the jazz drums cause or worsen arrhythmia?
3.
Do you have any recommendations for my symptoms?
Thank you very much for your patience in reading this, and I appreciate your help in answering my questions.

Hello: The autonomic nervous system is divided into two main systems: the sympathetic nervous system and the parasympathetic nervous system.
The sympathetic nervous system is active during the day when we are engaged in activities, while the parasympathetic nervous system takes over during nighttime rest.
These two systems have generally opposing functions and work in coordination with each other.
Although the autonomic nervous system is not directly controlled by the brain, it can be easily influenced by external stimuli, such as emotional changes, stress, and insomnia, which can lead to autonomic dysregulation.
The transition period between wakefulness and sleep is also when the regulation of the autonomic nervous system can fluctuate significantly, making it prone to dysregulation symptoms.
Playing the drums may potentially trigger palpitations, but this can vary from person to person; it would be advisable to observe whether discomfort occurs every time you play.
As for the tendency to feel dizzy, it may be related to changes in posture, so it's recommended to stay well-hydrated, which should help alleviate the symptoms.
If the symptoms persist frequently, please consult a cardiologist.

Dear Patient,
Thank you for your detailed inquiry regarding your arrhythmia and its relationship with your jazz drumming practice, particularly the symptoms you experience at night. I appreciate your patience and willingness to seek clarity on these matters. Let’s address your questions one by one.
1. Why do palpitations mostly occur at night?
Palpitations, or the sensation of feeling your heart beating, can be influenced by various factors, including the autonomic nervous system's activity. During the day, your body is typically in a more active state, with the sympathetic nervous system (responsible for the "fight or flight" response) being more dominant. At night, as you transition into sleep, the parasympathetic nervous system takes over, promoting relaxation and rest. This shift can lead to increased awareness of heartbeats, especially if there are underlying issues such as premature atrial contractions (PACs) or other arrhythmias. Additionally, factors such as anxiety, stress, and even the consumption of stimulants like caffeine can exacerbate these sensations, particularly when you are trying to wind down for sleep.
2. Can playing jazz drums exacerbate arrhythmia?
Playing a musical instrument like the jazz drums can indeed have an impact on your heart rhythm, but the effects can vary from person to person. Drumming is a physically engaging activity that can elevate your heart rate and increase adrenaline levels, especially in a high-energy genre like jazz. If you are experiencing palpitations after drumming, it might be due to the physical exertion and the excitement associated with playing. Moreover, exposure to loud sounds can also trigger stress responses in some individuals, potentially leading to increased heart rate or palpitations. It’s essential to monitor your symptoms closely and consider reducing the intensity or duration of your drumming sessions if you notice a consistent pattern of increased heart symptoms.
3. What recommendations do you have for my symptoms?
Given your history of arrhythmia and the symptoms you’ve described, here are some recommendations:
- Monitor Your Symptoms: Keep a diary of when your palpitations occur, their duration, and any associated activities (like drumming). This can help identify triggers.
- Hydration and Nutrition: Ensure you are well-hydrated and maintain a balanced diet. Dehydration can lead to electrolyte imbalances, which may exacerbate arrhythmias.
- Stress Management: Engage in relaxation techniques such as deep breathing, meditation, or yoga, which can help regulate your autonomic nervous system and potentially reduce palpitations.
- Limit Stimulants: Reduce or eliminate caffeine and other stimulants, especially in the hours leading up to bedtime.
- Consult Your Doctor: Since you have a history of heart issues, it’s crucial to maintain regular follow-ups with your cardiologist. If your symptoms persist or worsen, further evaluation may be necessary, including possibly wearing a Holter monitor to assess your heart rhythm over a more extended period.
In conclusion, while your arrhythmia may be benign, it’s essential to remain vigilant and proactive in managing your symptoms. Engaging in activities you enjoy, like drumming, can be beneficial for your mental health, but it’s crucial to balance this with your physical well-being. Please feel free to reach out to your healthcare provider for personalized advice tailored to your specific situation.
Take care, and I wish you all the best in your musical endeavors and health journey.
Sincerely,
