The foreskin has lesions that resemble herpes?
Hello doctor, on the night of August 12, I noticed peeling skin on my genital area and redness on the glans (the entire genital area appears red).
After showering and cleaning thoroughly, I retracted the foreskin and found white raised lesions at the opening of the foreskin (right at the position of the foreskin opening).
There are about 2-3 of these lesions clustered together; they are not painful or itchy, and they resemble herpes but do not look like blisters.
It has been six days since then, and there has been no improvement, but they have not grown larger either; however, the curvature of the lesions has become less pronounced.
Interestingly, they appeared suddenly in one day, as I did not notice them the day before after showering.
Additionally, I have been experiencing significant itching around my anal area for the past few days, which has caused some skin breakdown, but it has improved with medication.
I have not engaged in sexual activity for a very long time.
I would like to know if this could be due to an infection caused by having a long foreskin.
Thank you.

More Info
Based on your description, it seems you are experiencing a combination of symptoms that could be indicative of a few different conditions affecting the genital area. The redness and peeling of the skin on the glans (the head of the penis) and the presence of white bumps under the foreskin could suggest a few possibilities, including balanitis, a fungal infection, or even a sexually transmitted infection (STI), although you mentioned that you have not had sexual activity for a long time.
1. Balanitis: This is an inflammation of the glans and can be caused by poor hygiene, irritants, or infections. Symptoms typically include redness, swelling, and sometimes discharge. The white bumps you describe could be a result of irritation or an infection. If the area is not cleaned properly, it can lead to an overgrowth of yeast or bacteria, especially if moisture is trapped under the foreskin.
2. Fungal Infection: Conditions like candidiasis (yeast infection) can also cause redness and white patches. This is more common in individuals with diabetes or those who are immunocompromised. The itching you mentioned around the anal area could also be related to a yeast infection, as these infections can spread to nearby areas.
3. Sexually Transmitted Infections (STIs): Although you have not been sexually active recently, it is still important to consider STIs, especially if you have had any sexual partners in the past. Some STIs can present with similar symptoms, including redness, bumps, and irritation.
4. Other Conditions: Conditions such as lichen sclerosus or psoriasis can also cause similar symptoms, though these are less common. Lichen sclerosus, for instance, can lead to white patches and skin changes on the genital area.
Given that your symptoms have persisted for six days without improvement, it is crucial to seek medical attention. A healthcare provider can perform a physical examination and may recommend tests such as a swab of the affected area to identify any underlying infections. They may also suggest a urine test to rule out any urinary tract infections, especially since you have mentioned previous urinary symptoms.
In the meantime, here are some general recommendations:
- Maintain Hygiene: Gently clean the area with mild soap and water. Avoid harsh soaps or irritants that could exacerbate the condition.
- Avoid Irritants: Refrain from using scented products or lotions in the genital area, as these can cause further irritation.
- Loose Clothing: Wear loose-fitting underwear and clothing to allow the area to breathe and reduce moisture buildup.
- Avoid Sexual Activity: Until you have a clearer understanding of the condition, it may be wise to avoid sexual activity to prevent further irritation or potential transmission of an infection.
In conclusion, while there are several potential causes for your symptoms, a proper diagnosis from a healthcare professional is essential for appropriate treatment. Do not hesitate to reach out to a urologist or your primary care physician for an evaluation. Early intervention can help prevent complications and promote healing.
