A sudden swelling appears on the finger, and it hurts when pressed?
Hello doctor, I have been experiencing a long-term issue with my thumb that resembles paronychia.
After applying the previously prescribed medication today, I wrapped my finger up, and about two to three hours later, when I unwrapped it, I noticed that the area next to the nail was swollen and had a white patch.
It is quite painful when pressed.
I am unsure what this issue could be.
If further diagnosis is needed, should I still visit a dermatologist? Thank you!

Dr. Wu Yuxin reply Dermatology
The diagnosis cannot be confirmed because the lesion itself has not been observed; therefore, treatment by a dermatologist is appropriate.

More Info
Sudden finger swelling and pain can be concerning, especially when it follows a specific action, such as applying medication and wrapping the finger. Based on your description, it seems that you might be experiencing a localized inflammatory response, possibly due to a condition known as paronychia, which is an infection or inflammation of the skin around the nail. This can occur after trauma or irritation, such as wrapping the finger tightly or applying certain topical medications.
The white swelling you described could indicate fluid accumulation, which might be due to inflammation or infection. If the area is painful to the touch, this further supports the possibility of an inflammatory process. Other potential causes for sudden swelling and pain in the finger could include:
1. Infection: As mentioned, paronychia is a common infection that can occur around the nail. It can be caused by bacteria or fungi and may require treatment with antibiotics or antifungal medications.
2. Allergic Reaction: If the medication you applied contains ingredients that your skin is sensitive to, it could lead to localized swelling and pain. Allergic reactions can manifest as redness, swelling, and pain in the affected area.
3. Trauma: If there was any recent trauma to the finger, even if it seemed minor, it could lead to swelling and pain. This could be due to a bruise or a more significant injury that might not be immediately apparent.
4. Gout or Arthritis: Although less common, conditions like gout can cause sudden swelling and pain in the joints, including those in the fingers. This is usually accompanied by redness and warmth in the affected area.
5. Tendonitis: Inflammation of the tendons in the finger can also cause pain and swelling, particularly if there has been repetitive use or strain.
Given that you have a history of similar issues with your thumb, it is advisable to seek medical attention. A visit to a dermatologist or a hand specialist would be appropriate, as they can provide a thorough examination and determine the underlying cause of your symptoms. They may perform tests, such as a culture of any discharge, imaging studies, or a physical examination to assess the extent of the swelling and pain.
In the meantime, you can take some general measures to alleviate discomfort:
- Rest: Avoid using the affected finger as much as possible to prevent further irritation.
- Ice: Applying ice wrapped in a cloth to the swollen area can help reduce swelling and numb the pain.
- Elevation: Keeping the hand elevated can also help reduce swelling.
- Over-the-counter Pain Relief: Non-steroidal anti-inflammatory drugs (NSAIDs) like ibuprofen can help manage pain and inflammation.
In conclusion, while it may not be an emergency, it is essential to have your symptoms evaluated by a healthcare professional to ensure proper diagnosis and treatment. Early intervention can prevent complications and promote healing.
