After applying medication for demodex mites, my face feels burning and numb?

Hello, doctor.
I would like to inquire about a recent facial infection caused by Demodex mites.
I started applying Permethrin 5% w/w cream in the morning, and throughout the day, my face feels hot and uncomfortable, but it's bearable.
However, when I washed my face in the evening, I experienced a stinging sensation, and the area around my eyes and extending to my cheeks feels numb, particularly around the eyes.
This has been ongoing for about four hours now.
I would like to know if this is a normal side effect of the cream.
Thank you.

Ms. Chen, 20~29 year old female. Ask Date: 2022/09/16

Stop the medication first, and then have the treating physician reassess the actual situation.

Permethrin cream is commonly used to treat infestations caused by Demodex mites, which can lead to conditions such as rosacea. While it is effective in managing these infestations, it is important to be aware of potential side effects that may occur during treatment.

The symptoms you are experiencing, such as burning, discomfort, and a tingling or numb sensation around the eyes and cheeks, can be indicative of the skin's reaction to the active ingredients in Permethrin. Burning sensations are not uncommon when applying topical treatments, especially those containing insecticides or antiparasitic agents like Permethrin. This is often due to the skin's sensitivity to the formulation, particularly if there are pre-existing conditions such as rosacea or dermatitis.

In general, the side effects of Permethrin cream may include:
1. Burning or Stinging Sensation: This is a common reaction, especially during the initial applications. It may subside as your skin adjusts to the treatment.

2. Redness or Irritation: Some users may experience localized redness or irritation at the application site. This can be exacerbated if the skin is already inflamed or sensitive.

3. Dryness or Peeling: The use of Permethrin can lead to dryness, which may result in peeling skin. This is particularly relevant for individuals with sensitive skin or those who are already experiencing dryness due to other skin conditions.

4. Numbness or Tingling: As you described, some users report a tingling or numb sensation, particularly around sensitive areas such as the eyes. This could be a reaction to the cream or an indication of irritation.

5. Allergic Reactions: Although rare, some individuals may experience allergic reactions to Permethrin, which could manifest as severe itching, swelling, or hives. If you notice any of these symptoms, it is crucial to seek medical attention immediately.

Given that you have experienced these symptoms for about four hours, it is advisable to monitor your condition closely. If the burning sensation persists or worsens, or if you develop additional symptoms such as swelling or severe discomfort, it would be prudent to discontinue use and consult a healthcare professional. They may recommend an alternative treatment or provide guidance on managing the side effects.

In the meantime, to alleviate discomfort, consider the following measures:
- Cool Compress: Applying a cool, damp cloth to the affected areas can help soothe irritation and reduce the burning sensation.

- Moisturizers: Use a gentle, fragrance-free moisturizer to help combat dryness and irritation. Avoid products with alcohol or strong fragrances, as these can exacerbate sensitivity.

- Avoid Eye Contact: Be cautious to avoid applying the cream too close to the eyes, as this area is particularly sensitive. If you accidentally get the cream in your eyes, rinse thoroughly with water.

- Consult Your Doctor: If symptoms do not improve or if you have concerns about the side effects, reach out to your healthcare provider. They can assess your situation and determine whether the treatment should be adjusted.

In conclusion, while some discomfort is expected with the use of Permethrin cream, it is essential to differentiate between normal side effects and signs of a more serious reaction. Your healthcare provider is the best resource for managing your treatment and addressing any concerns you may have.
