Delayed ejaculation (male)
Hello doctor, I started masturbating in middle school, and by high school, I was doing it once a day, sometimes 2 to 3 times, and this has continued until now.
Recently, during sexual intercourse with my partner, which lasted over half an hour on 3 or 4 occasions, I still couldn't ejaculate.
Although the experience was pleasurable, I didn't feel the urge to ejaculate, and my erection would gradually soften, although I could still manage to continue.
In the end, I could only ejaculate through manual stimulation.
I'm unsure if this issue is related to long-term masturbation, where I have become accustomed to the pressure and speed of using my hand, leading to decreased sensitivity, or if there are other underlying problems.

It may be psychogenic (related to psychological factors).
It is recommended to consult a physician at the outpatient clinic for further evaluation and examination.

More Info
Delayed ejaculation (DE) is a condition where a man has difficulty achieving ejaculation during sexual intercourse, despite the ability to do so during masturbation. This can be a source of frustration and anxiety for both partners, and understanding the underlying causes is crucial for finding effective solutions.
Causes of Delayed Ejaculation
1. Psychological Factors: One of the most common causes of delayed ejaculation is psychological stress or anxiety. Performance anxiety, fear of intimacy, or past negative sexual experiences can create a mental block that inhibits ejaculation. In your case, the pressure to perform and the anxiety about not being able to ejaculate may exacerbate the problem.
2. Physical Factors: While less common, physical issues can also contribute to DE. These may include hormonal imbalances, neurological disorders, or side effects from medications. For instance, certain antidepressants, particularly selective serotonin reuptake inhibitors (SSRIs) like Escitalopram, can lead to delayed ejaculation as a side effect.
3. Habituation to Stimulation: As you mentioned, long-term masturbation habits can lead to a decrease in sensitivity. If you have conditioned your body to respond to specific types of stimulation (like the pressure and rhythm of your hand), it may become more challenging to achieve the same response during partnered sexual activity. This is often referred to as "sensitization," where the body becomes accustomed to a particular stimulus.
4. Relationship Dynamics: The dynamics of your relationship with your partner can also play a role. If there are unresolved issues or lack of communication regarding sexual needs and preferences, this can create additional stress that impacts sexual performance.
Solutions and Recommendations
1. Communication with Your Partner: Openly discussing your concerns with your partner can help alleviate some of the pressure you feel. Understanding that this is a shared issue can foster intimacy and reduce anxiety.
2. Relaxation Techniques: Engaging in relaxation techniques such as deep breathing, mindfulness, or yoga can help reduce anxiety levels. The goal is to create a more relaxed state during sexual activity, which may facilitate ejaculation.
3. Variety in Stimulation: Experimenting with different types of stimulation during intercourse can help. This might include varying the pace, rhythm, and positions to find what feels best for both you and your partner.
4. Limit Masturbation: If you believe that your masturbation habits are contributing to the issue, consider reducing the frequency or changing your technique. This may help increase sensitivity and responsiveness during sexual encounters.
5. Professional Help: If the problem persists, it may be beneficial to consult a healthcare professional, such as a urologist or a sex therapist. They can provide a thorough evaluation to rule out any underlying medical conditions and offer tailored treatment options.
6. Medication Review: If you are taking any medications, particularly SSRIs, discuss with your doctor the possibility of adjusting your dosage or switching to a different medication that may have fewer sexual side effects.
Conclusion
Delayed ejaculation can be a complex issue influenced by a combination of psychological and physical factors. Understanding the root causes is essential for finding effective solutions. By fostering open communication with your partner, exploring different stimulation techniques, and seeking professional guidance if necessary, you can work towards overcoming this challenge. Remember, sexual health is an important aspect of overall well-being, and addressing these concerns is a positive step towards a fulfilling sexual relationship.
