There was no bleeding at the injection site after receiving the COVID-19 vaccine?
Hello Doctor, I have received three doses of the BNT vaccine.
After getting my third dose yesterday, I noticed that the cotton ball applied to the injection site did not have any blood on it (I did not press on the cotton ball because the nurse advised against it).
I had the same situation after my first vaccination, with only the second dose showing some blood, which was also the dose that caused the most severe reaction.
Is it normal to not have any bleeding in this situation? Am I sure that I actually received the vaccine? Currently, I only have arm pain and no other symptoms (similar to my first dose).
Thank you!

Hello: The COVID-19 vaccine is administered via intramuscular injection, as muscle tissue is highly vascularized.
Therefore, during the injection, it is possible to damage microvessels, leading to bleeding.
Whether there is bleeding at the injection site or not is a normal occurrence and does not correlate with the administration of the vaccine.
Additionally, the more severe reaction you experienced after the second dose is likely related to the BNT vaccine.
Most individuals receiving mRNA vaccines (such as Moderna or BNT) typically experience more pronounced reactions after the second dose.

It is completely normal not to see any bleeding after receiving a COVID-19 vaccine injection, such as the BNT (BioNTech/Pfizer) vaccine. The absence of blood at the injection site does not indicate that the vaccine was not administered or that it was ineffective. Vaccines, including COVID-19 vaccines, are typically injected into the muscle (intramuscular injection), which is a highly vascularized tissue. This means that while there is a possibility of hitting a small blood vessel during the injection, it is not guaranteed that bleeding will occur every time.
When a vaccine is injected, the needle may or may not puncture a small blood vessel. If it does, you might see some blood on the cotton ball or gauze applied to the injection site. However, if the needle does not hit a blood vessel, there will be no bleeding, and this is perfectly normal. The fact that you experienced bleeding during your second dose but not during the first and third doses is also common and can vary from one injection to another based on the technique used, the location of the injection, and individual anatomical differences.
Regarding your concern about whether you received the vaccine, rest assured that the presence or absence of bleeding does not correlate with the effectiveness of the vaccine. The primary indicators of vaccination are the administration of the vaccine itself and the subsequent immune response your body generates. The soreness in your arm is a common side effect of the vaccine and indicates that your body is responding to the injection. Other side effects can include fatigue, headache, muscle pain, chills, fever, and nausea, but these can vary significantly from person to person.
It is also worth noting that many people report experiencing more pronounced side effects after their second dose of an mRNA vaccine, such as BNT. This is thought to be due to the immune system's heightened response after the first exposure to the vaccine, which prepares the body for a stronger reaction upon subsequent doses.
In summary, not seeing blood after your COVID-19 vaccine injection is normal and does not imply that the vaccine was not administered. Your experience aligns with what many others have reported, and the soreness in your arm is a typical reaction. If you have any further concerns or experience unusual symptoms, it is always a good idea to consult with your healthcare provider for personalized advice and reassurance.
