Blood Test Results and Ear Pain: When to Seek Medical Advice - Internal Medicine

Please consult the doctor regarding blood draw and related issues?

I apologize for the interruption.
I was diagnosed about 3-4 months ago, and sometimes I feel like I have some lingering symptoms, so I thought I should get a check-up.
Last week, I made an appointment with a family medicine specialist, and today I had blood drawn.
I just saw the blood test results (I have a follow-up appointment next Saturday).
I would like to ask the doctor about my lymphocyte count, which sometimes ranges from 47 to 54, and has even been as high as 60 in the past.
The family medicine doctor referred me to a hematologist, and I have had three follow-up visits, with the count finally around 47.
However, the doctor told me that this total can fluctuate, and I am concerned if it could be something serious.
In the end, he reassured me not to worry, stating that my ratio might just be like that, and he calculated that my count is actually within the normal range.
He also mentioned that even a slight cold could affect it, so he did not order any further tests for monitoring.
However, I just noticed that it has increased to 54 again.
Recently, I have also had a bit of phlegm.
I would like to ask the doctor if this data is truly not a concern, as it seems that routine blood tests do not usually show such a variety of blood cell data.
Additionally, I would like to ask about a slight pain when pressing on the outer bottom part of my ear.
I recall that there was a time when it suddenly bled, possibly due to a pimple or from scratching my ear, but it healed after applying some medication.
However, today while washing my hair, I noticed that pressing on it causes slight pain.
I read online that it could be related to neck infections or even cancer...
I would like to know if I need to get this checked out, or if I am just overly worried and should not be too anxious.

Dr. Hong Yuanbin reply Internal Medicine

Hello, regarding the lymphocyte count, as your physician mentioned, it is true that various conditions can cause fluctuations in individual values.
Your results are within the normal range, so there is no need to worry.
As for the outer part of your ear, based on your description and photos, it does not appear to be a serious condition.
I recommend monitoring it unless you experience significant discomfort, in which case you should seek medical attention.
Thank you for your inquiry.

More Info

Understanding blood test results and ear pain can be quite concerning, especially when you have a history of health issues. Based on your description, it seems you have been monitoring your lymphocyte counts and experiencing some discomfort in your ear. Let's break down your concerns systematically.

Blood Test Results
You mentioned that your lymphocyte count has fluctuated between 47-60, with your recent count being 54. It's important to understand that lymphocyte counts can vary due to several factors, including infections, stress, and even the time of day when the blood is drawn. The normal range for lymphocytes typically falls between 20% to 40% of your total white blood cell count, but this can vary based on the laboratory and individual health conditions.

Your healthcare provider has reassured you that your counts are within a normal range and that fluctuations are common. The presence of a mild respiratory infection, as you mentioned, can indeed elevate lymphocyte counts as your body responds to the infection. If your doctor has not indicated any cause for concern, it is likely that your results are not indicative of a serious condition. However, if you continue to feel unwell or if your symptoms worsen, it would be prudent to follow up with your healthcare provider.

Ear Pain and Discomfort
Regarding the pain you are experiencing at the base of your outer ear, it is essential to consider a few possibilities. The fact that you have had a previous injury or irritation in that area (like a pimple or skin abrasion) could lead to localized inflammation or infection, which might explain the tenderness you are feeling.
Infections in the ear can sometimes present as pain or discomfort, and they can be caused by various factors, including:
1. External Ear Infection (Otitis Externa): Often caused by bacteria or fungi, this can occur after water exposure or trauma to the ear canal.

2. Middle Ear Infection (Otitis Media): This typically presents with more systemic symptoms like fever, but can also cause referred pain to the outer ear.

3. Skin Issues: Conditions like eczema or dermatitis can cause irritation and pain in the ear area.

While it is understandable to worry about more severe conditions such as cancer, the symptoms you describe—especially if they are localized and related to a previous injury—are more likely to be benign. However, if you notice any of the following, it would be wise to seek medical attention:
- Persistent pain that does not improve
- Swelling or redness around the ear
- Discharge from the ear
- Fever or systemic symptoms

When to Seek Medical Advice
Given your history and current symptoms, it is advisable to consult a healthcare provider if:
- The pain in your ear worsens or does not improve over a few days.

- You develop additional symptoms such as fever, hearing loss, or significant swelling.

- You have ongoing concerns about your blood test results or any new symptoms arise.

In summary, while your lymphocyte counts appear to be within a normal range and your ear pain may not be indicative of a serious condition, it is always best to err on the side of caution. Regular follow-ups with your healthcare provider and addressing any new or worsening symptoms promptly will help ensure your health remains a priority. Always trust your instincts—if something feels off, it’s worth getting checked out.
