Probability of Pregnancy
Hello Doctor, my menstrual period last month was from September 16 to 22, and it completely ended on the 22nd.
I had unprotected sexual intercourse from September 23 to 25, with external ejaculation, but during one instance, there was ejaculation inside the anus, and I was unsure if any semen came into contact with the vulva.
Around September 28 to 29, I experienced noticeable ovulation discomfort and slight bleeding.
About 8 to 9 days before my expected next period, around October 4 to 5, I began to feel significant breast tenderness.
I would like to ask how likely it is that I could be pregnant.
Thank you, Doctor.
Xin, 20~29 year old female. Ask Date: 2022/10/06

Hello, the questioner: There is a possibility of pregnancy even if ejaculation has not occurred during intercourse, as pre-ejaculatory fluid may contain sperm.
Additionally, early pregnancy does not always present specific symptoms, and subjective symptoms cannot be used to make a judgment.
It is recommended to check for pregnancy if your period is more than a week late.
Thank you! Wishing you peace and health.

More Info
When considering the chances of pregnancy after unprotected intercourse, several factors come into play, including the timing of ovulation, the nature of the sexual activity, and individual variations in menstrual cycles. Based on the information provided, let's break down the situation.
Your last menstrual period started on September 16 and ended on September 22. Given a typical cycle length of about 28 days, ovulation likely occurred around September 30 to October 1. Engaging in unprotected intercourse from September 23 to 25 places you just before your ovulation window, which can indeed lead to a higher chance of conception, especially if sperm is present in the reproductive tract during ovulation.
The fact that you experienced ovulation discomfort and slight spotting around September 28 to 29 further supports the likelihood of ovulation occurring around that time. Sperm can survive in the female reproductive tract for up to five days, which means that intercourse occurring just before ovulation can result in fertilization if sperm is still viable when the egg is released.
Additionally, you mentioned experiencing breast tenderness around October 4 to 5, which can be an early sign of pregnancy. However, breast tenderness can also occur due to hormonal changes associated with the menstrual cycle, so it is not a definitive indicator of pregnancy.
To assess your chances of pregnancy more accurately, consider the following points:
1. Timing of Intercourse: Since you had unprotected intercourse just before ovulation, this increases the likelihood of pregnancy. The closer the intercourse is to ovulation, the higher the chances of sperm meeting the egg.
2. Nature of Intercourse: You mentioned that there was ejaculation outside the vagina, but there was also a possibility of contact with the vaginal area. Pre-ejaculate fluid can contain sperm, which could lead to pregnancy if it comes into contact with the vaginal canal.
3. Menstrual Cycle Variability: Individual menstrual cycles can vary in length and regularity. If your cycle is longer or shorter than the average 28 days, this could affect the timing of ovulation and, consequently, your chances of conception.
4. Pregnancy Testing: If your next expected period is delayed, it would be advisable to take a home pregnancy test for confirmation. Most tests are reliable about a week after a missed period, as they detect the hormone hCG, which is produced after a fertilized egg implants in the uterus.
5. Consulting a Healthcare Provider: If you have concerns about your chances of pregnancy or if you experience unusual symptoms, consulting a healthcare provider can provide personalized insights and guidance.
In summary, based on the timing of your unprotected intercourse and the signs of ovulation, there is a reasonable chance of pregnancy. However, the only way to confirm pregnancy is through testing after a missed period. If you have further questions or concerns, consider reaching out to a healthcare professional for more tailored advice.
