Medication Effects: Managing Mental Health and Side Effects - Psychiatry

Hello, I would like to ask you about diseases and medication issues?

In 2019, I took Risperidone and felt better.
A year later, I switched to 803, and my mother said I still needed adjustments.
The new doctor prescribed Olanzapine, which made me feel miserable.
Then in October 2020, I was hospitalized and took Aripiprazole, and I was discharged in December.
By February 2021, I started feeling unhappy again, but the feeling was different from the unhappiness I experienced with Olanzapine.
I also experienced retrograde ejaculation (feeling of ejaculation without any discharge).
In November 2021, I was hospitalized again and switched to Lurasidone, which improved my mood.
Finally, in January 2022, I successfully ejaculated after a long time.
However, in February 2022, I felt unhappy again, and the feeling was different from the unhappiness I had with Olanzapine and Aripiprazole.
So now my combination is Risperidone and Aripiprazole.

I would like to ask:
1.
Can Aripiprazole cause me to be unable to ejaculate?
2.
Am I being hyperactive because I keep moving around?
3.
What medications can help me feel happier and more stable, and improve my cognitive function (I am currently working from home, and I need good mental and physical health to work outside)? I understand that I cannot rely solely on medication; I also need to exercise, eat healthy foods, and read.
Thank you, doctor, and I apologize for the trouble in asking you to reply.

I recommend that you: 1.
Discuss the above situation further with your attending physician, focusing on symptom management or determining if further examinations are necessary.
2.
Consider seeking treatment from other medical institutions if needed.
Thank you, and wishing you good health!

Managing mental health through medication can be a complex and challenging process, particularly when side effects arise. Your experience with various medications, including Risperidone (often referred to as "里斯必妥"), and the subsequent changes in your treatment regimen highlight the importance of understanding both the therapeutic effects and potential side effects of these medications.

1. Medication and Sexual Side Effects: Regarding your question about whether Anxiolytics (安立復) can cause issues with ejaculation, it is indeed possible. Many psychiatric medications, particularly those that affect serotonin levels, such as SSRIs (Selective Serotonin Reuptake Inhibitors) and some antipsychotics, can lead to sexual side effects, including delayed ejaculation or anorgasmia (inability to achieve orgasm). This is a common concern among patients and can significantly impact quality of life. If you suspect that your medication is causing these issues, it is crucial to discuss this with your healthcare provider. They may consider adjusting the dosage or switching to a different medication that has a lower risk of sexual side effects.

2. Hyperactivity and Restlessness: Your observation about feeling restless or constantly moving could be indicative of akathisia, a common side effect of certain antipsychotic medications. Akathisia is characterized by a feeling of inner restlessness and an uncontrollable need to be in constant motion. It can be distressing and may be mistaken for hyperactivity. If this is affecting your daily life, it is essential to communicate this to your doctor, who may adjust your medication or suggest additional treatments to alleviate these symptoms.

3. Finding the Right Medication: The quest for the right medication that provides stability and happiness can be a lengthy process. It is essential to work closely with your psychiatrist to find a combination that works for you. While medications can help manage symptoms, they are often most effective when combined with lifestyle changes, such as regular exercise, a healthy diet, and engaging in activities that promote mental well-being, such as reading or mindfulness practices.
4. The Role of Lifestyle Changes: As you mentioned, self-care is a critical component of managing mental health. Regular physical activity can improve mood and cognitive function, while a balanced diet can support overall brain health. Additionally, engaging in social activities and maintaining a routine can provide structure and purpose, which are beneficial for mental health.

5. Communication with Healthcare Providers: It is vital to maintain open communication with your healthcare providers about your experiences with medications. If you are experiencing side effects or if your mental health is not improving, do not hesitate to reach out for help. Your treatment plan may need adjustments, and your healthcare team is there to support you in finding the most effective approach.

In summary, managing mental health through medication requires careful consideration of both the benefits and potential side effects. It is crucial to work closely with your healthcare provider to find the right balance and to incorporate healthy lifestyle choices to support your mental well-being. Remember, you are not alone in this journey, and there are resources and support available to help you navigate these challenges.
