Should I Return to the Doctor If My Period Is Late After a Hormonal Injection? - Obstetrics and Gynecology

If it has been seven days since you received the menstrual induction injection and your period has not yet started, should you return for a follow-up appointment or wait a little longer?

My menstrual period was from September 10 to September 15.
In October, I only had a very light flow on October 9, and I also took a home pregnancy test on October 11, which showed one line.
Subsequently, I visited a gynecologist on October 13 for an ultrasound, and the doctor indicated that the endometrium appeared thick and seemed not to have shed.
I took another pregnancy test, which also showed one line.
Therefore, I received an injection to induce menstruation.
As of now (October 20), I still have not seen my period.
Should I seek medical attention immediately, or can I wait another week without being too concerned? Thank you for your response.

If your period is late after receiving a hormonal injection, it is important to monitor your situation closely and consider a few factors before deciding whether to return to the doctor. Hormonal treatments, such as progesterone injections, are often used to regulate menstrual cycles, especially in cases where there may be irregularities or delays. However, the response to these treatments can vary significantly among individuals.

In your case, you mentioned that your last menstrual period was from September 10 to September 15, and you experienced a very light flow on October 9 and 11. After visiting your gynecologist on October 13, you received an injection to induce menstruation due to a thickened endometrial lining, which suggests that your body was preparing for a period but did not shed the lining as expected. The fact that you have not yet seen your period following the injection is understandably concerning.

Generally, after receiving a hormonal injection, it can take some time for your body to respond. It is not uncommon for there to be a delay in menstruation, especially if your hormonal levels were already disrupted prior to treatment. In many cases, it may take a week or more for the effects of the injection to manifest as a menstrual period. Therefore, if you have not experienced any significant symptoms such as severe pain, heavy bleeding, or other alarming signs, it may be reasonable to wait another week before seeking further medical advice.

However, if you experience any of the following symptoms, it would be advisable to return to your doctor sooner rather than later:
1. Severe Abdominal Pain: This could indicate an issue such as an ectopic pregnancy or other complications that require immediate attention.

2. Heavy Bleeding: If you start to experience heavy bleeding that soaks through pads or tampons in an hour or less, you should seek medical help.

3. Signs of Pregnancy: If you suspect you might be pregnant despite the negative pregnancy test, it’s important to consult your doctor for further evaluation.

4. Persistent Symptoms: If you have other concerning symptoms, such as fever, chills, or unusual discharge, these could indicate an infection or other medical issue.

In summary, while it is often acceptable to wait a week after a hormonal injection to see if your period arrives, you should remain vigilant for any concerning symptoms. If you feel uneasy or if your symptoms change, do not hesitate to reach out to your healthcare provider for guidance. They can provide personalized advice based on your medical history and current situation. Always prioritize your health and well-being, and trust your instincts when it comes to seeking medical care.
