Loss of appetite
I have been experiencing a loss of appetite for two weeks.
It started with nausea due to lack of sleep, and since then, I have not felt hungry at all, even having a constant sensation of fullness.
My weight has dropped from 43 kg to 40 kg, and my BMI has fallen to just over 15.
I can go all day without eating and still not feel hungry.
My abdomen makes loud noises, but I am not having bowel movements.
Given that I have frequently stayed up late and had irregular sleep patterns over the past two to three years, could this be related to liver issues? This is the first time I have been unable to eat anything, and I even feel a sense of nausea.

Dr. Hong Yuanbin reply Internal Medicine
Hello, insufficient sleep can indeed affect appetite.
As for whether there are issues with the liver or other organs, such as the thyroid, a detailed examination may be necessary to determine that.
It is recommended to visit a family medicine clinic for a thorough evaluation.
Thank you for your inquiry.

Loss of appetite can be a concerning symptom that may arise from various underlying causes, and it is important to understand these potential reasons to address the issue effectively. In your case, the loss of appetite has persisted for two weeks, accompanied by significant weight loss, a decrease in BMI to a concerning level, and gastrointestinal symptoms such as abdominal rumbling without bowel movements.
Several factors can contribute to a loss of appetite, including psychological, physiological, and lifestyle-related issues. Given your history of sleep deprivation and irregular sleep patterns, it is possible that stress and anxiety could be playing a significant role in your current condition. Chronic stress can lead to hormonal imbalances that affect appetite regulation, often resulting in decreased hunger or feelings of fullness even when the body needs nourishment.
Additionally, your mention of experiencing nausea and a sensation of fullness without eating could indicate gastrointestinal disturbances. Conditions such as gastritis, gastroesophageal reflux disease (GERD), or even more serious issues like peptic ulcers can lead to similar symptoms. These conditions can be exacerbated by irregular eating habits, stress, and lack of sleep, which may have been part of your lifestyle for the past few years.
Another important consideration is the potential impact of your liver health. The liver plays a crucial role in metabolism and digestion, and chronic sleep deprivation and poor dietary habits can lead to liver dysfunction. Symptoms of liver issues can include changes in appetite, nausea, and abdominal discomfort. It would be prudent to consult a healthcare professional to evaluate your liver function through blood tests and possibly imaging studies.
Moreover, the significant weight loss you have experienced (from 43 kg to 40 kg) is alarming, especially with a BMI dropping below 18.5, which categorizes you as underweight. Being underweight can lead to a host of health problems, including weakened immune function, decreased muscle mass, and potential nutrient deficiencies. It is essential to address this weight loss promptly to prevent further health complications.
In terms of immediate steps, I recommend seeking medical attention as soon as possible. A healthcare provider can conduct a thorough assessment, including a physical examination and necessary laboratory tests, to identify any underlying medical conditions contributing to your symptoms. They may also evaluate your mental health, as conditions like depression or anxiety can significantly impact appetite and overall well-being.
In the meantime, try to maintain a regular eating schedule, even if you do not feel hungry. Small, nutrient-dense meals or snacks can help provide your body with the necessary energy and nutrients it needs. Staying hydrated is also crucial, especially if you are experiencing gastrointestinal symptoms.
In summary, loss of appetite can stem from various causes, including psychological stress, gastrointestinal issues, and potential liver problems. Given your symptoms and weight loss, it is vital to seek medical advice to determine the underlying cause and receive appropriate treatment. Taking proactive steps now can help you regain your appetite and improve your overall health.
