Questions about steroid eye drops?

Hello, Doctor Gao.
I have an allergic constitution and seborrheic dermatitis, which often leads to blepharitis.
Every time I visit an ophthalmologist, they prescribe Salon and Hu-Kang Vision (0.1% steroid) eye drops, but I mostly use antibiotic eye drops, only resorting to steroids when absolutely necessary.
I would like to ask why I experience temporary blurred vision (lasting 15 to 30 minutes) after using the purple Hu-Kang Vision drops.
Thank you.

Wanfu, 30~39 year old female. Ask Date: 2022/10/23

Dr. Gao Pilin reply Ophthalmology

Hello: It is normal for steroid solutions to appear as a suspension, which may contain fine white particles.

Steroid eye drops, such as the ones you mentioned (like the "護康視" or similar corticosteroid formulations), are commonly prescribed for various ocular conditions, particularly those involving inflammation, such as allergic conjunctivitis or blepharitis. While these medications can be very effective in reducing inflammation and alleviating symptoms, they can also lead to temporary side effects, including blurred vision.

The temporary blurriness you experience after using steroid eye drops can be attributed to several factors:
1. Viscosity of the Drops: Many steroid eye drops are formulated with a thicker consistency to enhance their retention on the ocular surface. This increased viscosity can cause a temporary blurring of vision as the drop spreads across the cornea and interacts with the tear film. The blurriness usually resolves as the drop is absorbed or as the tear film stabilizes.

2. Tear Film Disruption: Steroid drops can alter the composition of the tear film, which is crucial for maintaining clear vision. If the tear film is disrupted, it can lead to transient visual disturbances, including blurriness. This is particularly relevant if you have a pre-existing condition like dry eye or blepharitis, which can already compromise the stability of your tear film.

3. Pupil Dilation: Some steroid drops can cause mild dilation of the pupil, which may affect your ability to focus, especially in bright light or when looking at objects up close. This can lead to a sensation of blurriness until your eyes adjust.

4. Intraocular Pressure Changes: Although less common, prolonged use of steroid eye drops can lead to increased intraocular pressure, which can affect vision. However, this typically occurs with long-term use rather than the short-term application you described.

5. Corneal Sensitivity: If you have underlying conditions such as blepharitis or other forms of ocular surface disease, the application of any eye drop, including steroids, can temporarily affect corneal sensitivity and lead to visual disturbances.

Given that you have a history of allergic conditions and seborrheic dermatitis, it is essential to manage these underlying issues effectively. While steroid eye drops can provide relief, they should be used judiciously, as prolonged use can lead to complications such as cataract formation or glaucoma.

If you find that the blurriness persists beyond the typical duration of 15 to 30 minutes, or if you experience other symptoms such as increased redness, pain, or vision changes, it is crucial to follow up with your eye care provider. They may need to reassess your treatment plan or consider alternative therapies that might be better suited for your condition.

In addition to steroid drops, it is also advisable to maintain good eyelid hygiene, especially if you have blepharitis. Regular cleaning of the eyelid margins can help reduce inflammation and improve overall eye comfort. Artificial tears can also be beneficial in managing dryness and maintaining a stable tear film, particularly if you are using steroid drops that may contribute to dryness.

In summary, while temporary blurriness after using steroid eye drops is a common and generally benign side effect, it is essential to monitor your symptoms and communicate with your healthcare provider to ensure optimal management of your ocular health.
