Menstrual period with continuous bleeding?

Hello, doctor.
My menstrual cycle is approximately 25 days, and it has been quite regular until recently.
My last menstrual period lasted from August 29 to September 10, which was unusually long, so I visited a gynecologist and took hemostatic medication.
Then, my next period started on October 25 and ended on October 29.
However, less than five days later, my period returned on November 3.
This time, the flow was minimal, with only some light brown discharge on my panty liner.
I only noticed blood when using the restroom or wiping.
There were also some blood clots during this time.
What could this situation indicate? Should I consult a gynecologist or a urologist? I would appreciate your assistance in clarifying this.
Thank you.

Hello, it could be cystitis.
I recommend seeing a urologist.
Wishing you good health.

Irregular menstrual cycles can be concerning, especially when there are significant changes in the frequency, duration, and characteristics of menstrual bleeding. Based on your description, it seems that you have experienced a notable shift in your menstrual pattern, which warrants further investigation.

1. Understanding Your Symptoms: Your menstrual cycle has historically been regular, with a cycle length of about 25 days. However, you have recently experienced prolonged bleeding from August 29 to September 10, followed by a cycle starting on October 25 and then another on November 3. The presence of brownish discharge and blood clots can indicate several things, including hormonal imbalances, uterine fibroids, polyps, or even endometriosis. The brown color of the blood often signifies older blood that has taken longer to exit the body, which can be normal but may also indicate an underlying issue if it occurs frequently.

2. When to Seek Medical Advice: Given the changes in your menstrual cycle and the symptoms you are experiencing, it is advisable to consult a gynecologist (婦產科) rather than a urologist (泌尿科). A gynecologist specializes in female reproductive health and can provide a comprehensive evaluation of your menstrual irregularities. They may perform a pelvic exam, ultrasound, or other diagnostic tests to determine the cause of your symptoms.

3. Potential Causes: Irregular menstrual cycles can be caused by various factors, including:
- Hormonal Imbalances: Fluctuations in hormones such as estrogen and progesterone can lead to irregular cycles. Conditions like polycystic ovary syndrome (PCOS) are common culprits.

- Uterine Conditions: Fibroids or polyps can cause prolonged bleeding and irregular cycles. These are non-cancerous growths in the uterus that can affect menstrual flow.

- Endometriosis: This condition occurs when tissue similar to the lining of the uterus grows outside it, leading to pain and irregular bleeding.

- Stress and Lifestyle Factors: High stress levels, significant weight changes, and excessive exercise can also disrupt your menstrual cycle.

4. What to Expect During Your Visit: When you see a gynecologist, be prepared to discuss your menstrual history in detail, including the duration and flow of your periods, any associated symptoms (such as pain or discomfort), and any other health issues you may have. The doctor may recommend blood tests to check hormone levels, an ultrasound to visualize the reproductive organs, or a biopsy if there are concerns about the uterine lining.

5. Management and Treatment: Depending on the diagnosis, treatment options may vary. If hormonal imbalances are identified, hormonal therapy may be suggested. If fibroids or polyps are present, they may need to be monitored or surgically removed. Lifestyle changes, such as stress management and maintaining a healthy weight, can also help regulate your cycle.

In conclusion, while some irregularities in menstrual cycles can be benign, the changes you are experiencing—especially the prolonged bleeding and the presence of blood clots—should be evaluated by a healthcare professional. Seeking timely medical advice will help ensure that any underlying issues are addressed and managed appropriately.
