Testicular issues

Hello, Doctor! I have a question that has been troubling me.
I have health anxiety, so I tend to be more sensitive to changes in my body.
Recently, while taking a shower, I noticed an oval-shaped hard lump below my right testicle (there seems to be a smaller one on the left side as well).
Out of curiosity, I searched online and found that it could be a sign of testicular cancer, which has made me very anxious.
Since I live in a rural area, there are no urology clinics nearby, but there is a family medicine clinic.
I visited the family medicine clinic for an examination, and the doctor performed a physical exam.
He said the size was normal and did not feel any hard lump, but advised me to monitor it if it grows.
About two weeks later, I noticed that the lump is more noticeable after showering at night or sometimes after ejaculation, but by the morning, I can no longer feel it.
Because of my anxiety, I went to a second family medicine clinic (with a different doctor), and after a physical exam, he also did not find any hard lump.
The family medicine doctor told me that if it were testicular cancer, I would be able to feel it regardless of the time of day, and it wouldn't be there at night and gone by morning.
However, I am still very anxious, so I would like to ask the doctor if I am being overly suspicious.
Should I consider getting an ultrasound at the hospital? Thank you, Doctor!

Hello: If you are certain that you have felt something, it is advisable to consult a urologist at a hospital.
Of course, testicular cancer is not common, so there is no need to worry too much.
Wishing you good health.

It's completely understandable to feel anxious about changes in your body, especially when you have a history of health anxiety. The concerns you've raised regarding the lump you've felt in your testicle are valid, and it's important to address them appropriately.

First and foremost, it's essential to recognize that testicular lumps can arise from various causes, and not all of them are serious. Common benign conditions include epididymal cysts, varicoceles, or even normal anatomical variations. However, it's also true that testicular cancer can present as a lump or swelling, which understandably raises concern.

You mentioned that you felt a lump under your right testicle and that it seems to be more noticeable at certain times, such as after bathing or after ejaculation. The fact that the lump is not consistently palpable (i.e., you can't feel it in the morning) is a reassuring sign. Testicular cancer typically presents as a persistent lump that does not fluctuate in size or disappear. However, the best course of action is to trust your instincts and seek further evaluation if you remain concerned.

Since you've already consulted two different family medicine doctors who performed physical examinations and did not find any concerning masses, it may be beneficial to consider their advice. They indicated that if it were cancer, you would likely feel the lump consistently. However, if your anxiety persists, it might be worthwhile to pursue an ultrasound. An ultrasound is a non-invasive imaging test that can provide a clearer picture of what's happening in the scrotum and can help rule out any serious conditions, including tumors.

In terms of when to seek medical advice, here are some guidelines:
1. Persistent Changes: If you notice any persistent lumps or changes in the size or shape of your testicles, it’s important to seek medical evaluation.

2. Accompanying Symptoms: If you experience any additional symptoms such as pain, swelling, changes in the skin of the scrotum, or any unusual discharge, you should consult a healthcare provider promptly.

3. Anxiety and Health Concerns: If your anxiety about your health is affecting your daily life, it may be beneficial to speak with a mental health professional. They can help you develop coping strategies for managing health anxiety.

4. Follow-Up: If your anxiety continues despite reassurance from healthcare providers, consider scheduling a follow-up appointment for an ultrasound. This can provide peace of mind and help you feel more in control of your health.

In conclusion, while it’s good to be vigilant about your health, it’s also important to balance that vigilance with reassurance from medical professionals. If you feel that an ultrasound would help alleviate your concerns, don’t hesitate to pursue that option. Your health and peace of mind are paramount. Remember, it’s always better to err on the side of caution when it comes to health matters.
