The penis and scrotum feel cold?

Hello doctor, the weather has been getting cooler lately, and I feel that my penis and scrotum are cold all day long, without any shrinkage; it's not a case of heat expansion and cold contraction.
This didn't happen before.
My sexual function is inconsistent; sometimes it's normal, and other times it becomes soft and cannot achieve an erection.
What could this situation be? Do I need to have my prostate or any other areas examined? (I previously experienced symptoms of urinary difficulties, lower back discomfort, and numbness in the buttocks.)

A Cheng, 30~39 year old female. Ask Date: 2022/11/17

Dr. Du Shixi reply Urology

Hello: The sensation of coldness may be related to vascular circulation.
It is important to pay attention to overall health, such as exercise and diet.
If sexual function is not satisfactory, it is advisable to seek medical attention for necessary examinations.
Wishing you good health.

The sensation of coldness in the penis and scrotum, particularly during colder weather, can be attributed to several physiological and medical factors. As temperatures drop, blood flow to the extremities, including the genital area, may decrease, leading to a feeling of coldness. This is a normal response of the body to conserve heat and maintain core temperature. However, when this sensation is accompanied by other symptoms such as erectile dysfunction, urinary difficulties, and discomfort in the lower back or buttocks, it may indicate underlying health issues that warrant further investigation.

One potential cause of the cold sensation in the genital area could be related to vascular health. Conditions that affect blood circulation, such as peripheral artery disease (PAD), can lead to reduced blood flow to the penis and scrotum, resulting in a cold feeling. Additionally, diabetes and hypertension can also impact vascular health and may contribute to erectile dysfunction. If you have a history of these conditions, it is essential to discuss them with your healthcare provider.

Another factor to consider is nerve health. The sensation of coldness, along with symptoms like numbness in the buttocks, may suggest nerve compression or damage. Conditions such as sciatica, herniated discs, or even diabetes-related neuropathy can lead to altered sensations in the lower body. If you are experiencing persistent numbness or discomfort, a thorough neurological examination may be necessary.

Hormonal imbalances, particularly involving testosterone, can also affect sexual function and sensation. Low testosterone levels can lead to erectile dysfunction and changes in libido. If you suspect hormonal issues, a blood test to evaluate hormone levels may be beneficial.

Given your history of urinary difficulties and discomfort in the lower back, it would be prudent to consider a comprehensive evaluation of your prostate health. Conditions such as benign prostatic hyperplasia (BPH) or prostatitis can lead to urinary symptoms and may also impact sexual function. A urologist can perform the necessary tests, including a digital rectal exam and possibly a prostate-specific antigen (PSA) test, to assess prostate health.

In terms of immediate solutions, ensuring adequate warmth during colder months is essential. Wearing appropriate clothing, including thermal underwear, can help maintain warmth in the genital area. Additionally, engaging in regular physical activity can improve circulation and overall vascular health, which may alleviate some of the cold sensations.

If the cold sensation persists or worsens, or if you experience significant changes in sexual function, it is crucial to seek medical advice. A healthcare provider can perform a detailed assessment, including blood tests, imaging studies, and possibly referrals to specialists such as a urologist or neurologist, to determine the underlying cause and appropriate treatment options.

In summary, while the sensation of coldness in the penis and scrotum can be a normal response to colder weather, the accompanying symptoms you describe suggest that a more thorough evaluation is warranted. Addressing any underlying health issues, maintaining warmth, and consulting with healthcare professionals will be essential steps in managing your symptoms effectively.
