Epididymal issues
Hello, Doctor.
Five years ago, I went to the hospital for an ultrasound due to discomfort in my testicles, and it was found that I had a varicocele on the left side, but it wasn't severe, so I was advised to just monitor it.
Recently, I've been experiencing discomfort again, so I went for another check-up.
The condition on the left side is about the same, but now there is also some varicocele on the right side.
Additionally, the doctor mentioned that compared to five years ago, the right epididymis is not as full or "nice" as it used to be.
However, the doctor said that as long as I haven't experienced any trauma, it shouldn't be a major issue, and I should just keep monitoring it.
I would like to ask, is it normal for the epididymis to undergo changes? Is it related to development and the frequency of ejaculation? Thank you for your patience in reading and responding, Doctor.
Thank you.

Understanding changes in the epididymis, particularly in the context of varicocele and its effects on male reproductive health, is crucial for maintaining overall well-being. The epididymis is a coiled tube located at the back of each testis, responsible for storing and maturing sperm. Changes in its structure or function can indicate underlying issues that may affect fertility and sexual health.
In your case, you mentioned having a left-sided varicocele, which is a dilation of the veins within the scrotum. Varicoceles are relatively common and can lead to discomfort, but they are also associated with potential fertility issues. The fact that your left side remains similar to five years ago while the right side has developed some varicosity is noteworthy. Varicoceles can affect the temperature regulation of the testes, which is critical for sperm production. Elevated temperatures can impair sperm development and lead to changes in the epididymis.
The observation that your right epididymis is not as "full" or "beautiful" as it once was could be indicative of changes in sperm storage or maturation. This can occur due to various factors, including hormonal changes, blood flow issues, or even the presence of a varicocele. While the doctor mentioned that if you haven't experienced trauma, it should not be a significant concern, it is essential to monitor any changes closely.
Regarding your question about whether changes in the epididymis are normal, it is important to understand that some degree of variation can occur over time due to factors such as age, hormonal fluctuations, and lifestyle changes. However, significant changes or discomfort should always be evaluated by a healthcare professional.
As for the relationship between epididymal changes, development, and ejaculation frequency, there is some evidence to suggest that sexual activity can influence sperm health and the condition of the epididymis. Regular ejaculation may help maintain the health of the epididymis by promoting the clearance of older sperm and allowing for the maturation of new sperm. Conversely, infrequent ejaculation can lead to the accumulation of older sperm, which may affect the overall health of the sperm and the epididymis.
In summary, while some changes in the epididymis can be normal, especially in the context of varicocele, it is essential to remain vigilant about any discomfort or significant changes. Regular follow-ups with your healthcare provider are crucial to monitor your condition and address any potential issues early on. If you experience increased discomfort, changes in sexual function, or concerns about fertility, do not hesitate to seek further evaluation. Maintaining a healthy lifestyle, including regular exercise, a balanced diet, and managing stress, can also contribute positively to reproductive health.
