Endocrine disorders or other causes?
Hello Doctor,
On November 5th, my girlfriend and I engaged in external genital rubbing (she is a virgin) for about 3 to 5 minutes.
At that time, she had just finished her menstrual period the day before.
Then on November 18th, she told me that while taking a shower, she noticed a brown discharge that resembled blood, but it was not yet time for her period.
After researching online, it seemed similar to ovulation bleeding, and she mentioned that November 18th was the last day of her ovulation period.
The next morning (November 19th), she said it had stopped.
I then touched her with my fingers, inserting them inside, and halfway through, I noticed brown bleeding.
On November 20th, she reported that in the morning it was red, and in the afternoon it was brown.
On November 21st, she woke up with brown discharge again but no red blood.
My girlfriend mentioned that she hasn't experienced breast tenderness.
I found information online about hormonal imbalances, pregnancy, etc.
Could you please advise what might be happening? Thank you, Doctor.

Hello: 1.
You should take a pregnancy test first.
2.
If you are not pregnant, it could be due to hormonal imbalance or ovulation bleeding.

Abnormal bleeding can be a concerning issue for many women, and understanding the underlying causes is essential for proper management. In your case, the timeline and symptoms you've described suggest a few possibilities, including hormonal fluctuations, ovulation bleeding, or other factors.
First, let's clarify what abnormal bleeding is. Abnormal bleeding refers to any bleeding that occurs outside of a woman's regular menstrual cycle. This can include bleeding between periods, heavy menstrual bleeding, or bleeding after sexual intercourse. In your girlfriend's case, the coffee-colored discharge and subsequent bleeding could be indicative of several things.
1. Ovulation Bleeding: As you mentioned, your girlfriend experienced bleeding around the time of ovulation. Ovulation bleeding is a common phenomenon that occurs in some women when the ovary releases an egg. This can cause a slight drop in estrogen levels, leading to a small amount of bleeding or spotting. This type of bleeding is usually light and short-lived, often appearing as pink or brown discharge. Given that your girlfriend's bleeding occurred shortly after her menstrual period and coincided with her ovulation, this could very well be the cause.
2. Hormonal Imbalance: Hormonal fluctuations can lead to abnormal bleeding. Conditions such as polycystic ovary syndrome (PCOS), thyroid disorders, or stress can disrupt the normal hormonal balance, leading to irregular bleeding patterns. If your girlfriend has a history of irregular cycles or other symptoms of hormonal imbalance, this could be a contributing factor.
3. Injury or Irritation: The act of digital penetration (using fingers) can sometimes cause minor trauma to the vaginal walls or cervix, especially if there was insufficient lubrication or if the penetration was forceful. This could lead to bleeding, which might appear as coffee-colored discharge initially and then change to red. If the bleeding is light and resolves quickly, it is usually not a cause for concern.
4. Pregnancy: Although you mentioned that your girlfriend is a virgin, it’s important to note that any form of sexual activity can potentially lead to pregnancy if sperm comes into contact with the vaginal area. However, given that she has not had penetrative intercourse, the likelihood of pregnancy is low. If there is any doubt, a pregnancy test can provide clarity.
5. Other Causes: Other potential causes of abnormal bleeding include infections, cervical polyps, or other gynecological issues. If the bleeding persists or is accompanied by other symptoms such as pain, fever, or unusual discharge, it would be wise for her to consult a healthcare provider for further evaluation.
In summary, the symptoms your girlfriend is experiencing could likely be attributed to ovulation bleeding, especially given the timing in relation to her menstrual cycle. However, hormonal imbalances or minor injuries from digital penetration could also be contributing factors. If the bleeding continues or worsens, or if there are any other concerning symptoms, it would be advisable for her to seek medical attention to rule out any underlying issues. Regular gynecological check-ups can also help monitor her reproductive health and address any concerns related to her menstrual cycle.
