Phimosis and excessive foreskin?

I am currently a ninth-grade student.
I feel that my foreskin is too long.
When not erect, my glans can be fully exposed, but the skin feels a bit long.
During the health check in middle school, they did not diagnose me with phimosis, but when I am erect, the glans cannot be fully exposed; I can see a little bit of the surface, but I cannot pull it down as it gets stuck.
However, I can still masturbate and have no issues with hygiene.
Should I consider getting a circumcision during the upcoming summer vacation? How long is the recovery time for circumcision? I want to do it during summer vacation so that my friends won't find out.

Hello: The circumcision procedure takes a short time, approximately 30-40 minutes with anesthesia.
As for wound care, it should heal in about 1-2 weeks if managed properly.
Wishing you good health.

Phimosis, a condition where the foreskin cannot be fully retracted over the glans (the head of the penis), can be a source of concern for many, especially during adolescence. In your case, it seems that you have a long foreskin that does not retract fully when you are erect, but you can expose the glans when not erect. This situation is not uncommon and can be classified as a mild form of phimosis.

Understanding Phimosis and Long Foreskin
1. Normal Anatomy: The foreskin is a fold of skin that covers the glans of the penis. In many males, the foreskin is naturally tight at birth, and it may take several years for it to become retractable. By the time boys reach puberty, most will have a fully retractable foreskin.

2. Phimosis: This condition can be physiological (normal in younger boys) or pathological (due to scarring or infection). If you can retract the foreskin when not erect but not when erect, it may indicate a tight foreskin that could lead to discomfort during sexual activity or hygiene issues in the future.

3. Symptoms and Concerns: You mentioned that you can still clean yourself properly and that you can achieve erections without pain. However, if you experience pain, difficulty during sexual activity, or recurrent infections, these could be signs that treatment is necessary.

Treatment Options
1. Conservative Management: In many cases, especially if there are no significant symptoms, conservative management may be sufficient. This can include gentle stretching exercises of the foreskin, which may help in some cases. Topical steroid creams can also be prescribed to help loosen the skin.

2. Circumcision: If conservative measures do not work or if you are experiencing significant issues, circumcision may be recommended. This surgical procedure involves the removal of the foreskin and is typically performed under local anesthesia.
3. Timing and Recovery: The recovery time for circumcision is usually about one to two weeks, during which you may experience some discomfort and swelling. Most individuals can return to normal activities, including school, within a week, but it’s advisable to avoid strenuous activities for a short period.

Considerations for Surgery
- Timing: If you are considering circumcision during your summer break, it is a practical choice as it allows for recovery time without the pressure of school activities.
- Privacy: Many people choose to have this procedure done discreetly, and it is understandable to want to avoid drawing attention from peers.

- Consultation: It is crucial to discuss your concerns with a healthcare provider, preferably a urologist. They can provide a thorough examination and help you weigh the pros and cons of circumcision based on your specific situation.

Conclusion
In summary, while your condition may not require immediate surgical intervention, it is essential to monitor any changes or discomfort. If you decide to pursue circumcision, planning it during the summer is a reasonable approach to ensure adequate recovery time. Always consult with a healthcare professional to discuss your symptoms, treatment options, and any concerns you may have regarding the procedure and its implications.
