Numbness in the right thumb and index finger after yoga?

Hello Doctor: A few days ago, I practiced yoga as usual, and this particular class focused more on wrist usage.
After the class, I felt numbness in my right thumb and index finger.
I thought it would go away in a few minutes, but it has been four days now, and the numbness persists.
There is no pain, and it does not affect my daily activities.
I do not experience any nighttime numbness, but I continuously feel numbness in my thumb.
However, I can use it normally, and there is no weakness.
Occasionally, when water from the shower hits my forearm (on the thumb side), I also feel some numbness.
Could you please advise what this might be? Thank you!

Dr. Jiang Junyi reply Neurology

Dear Ms.
Wang,
You mentioned experiencing numbness in your right thumb and index finger, which may warrant consideration of typical discomfort symptoms associated with cervical radiculopathy.
These symptoms can include pain in the shoulder or arm, as well as numbness, tingling, and weakness in the palm and fingers.
In particular, compression of the sixth and seventh cervical vertebrae may lead to symptoms of numbness and weakness in the fingertips.
Peripheral nerve issues are typically assessed using electrophysiological studies to aid in diagnosis.
Nerve conduction studies measure the conduction velocity and waveform of large fiber nerves.
Electromyography (EMG) assesses muscle responses in the areas supplied by the affected nerves.
Clinically, cervical traction may be employed to alleviate symptoms, commonly used for cervical degenerative joint disease, herniated discs (cervical osteophytes), with associated nerve root compression, as well as cervical injuries without fractures or joint instability that compress the nerves.
Prior to traction, an X-ray examination is necessary to confirm joint stability.
I recommend that you visit a neurology outpatient clinic for further evaluation and follow-up.
Thank you for your inquiry, and feel free to discuss any related questions!
Wishing you good health and peace,
Dr.

More Info

Persistent numbness in the thumb and index finger, particularly after a yoga class that emphasizes wrist use, can be concerning. While it’s good to hear that you’re not experiencing pain or significant functional impairment, the continued numbness for four days warrants attention. Here are some potential causes and considerations based on your symptoms.

Possible Causes
1. Nerve Compression: The most likely explanation for your symptoms is some form of nerve compression, particularly involving the median nerve, which innervates the thumb and index finger. This can occur due to prolonged positions during yoga that place stress on the wrist or forearm. If your wrist was flexed or extended in an awkward position for an extended period, it could lead to temporary compression of the nerve.

2. Repetitive Strain Injury: Engaging in activities that require repetitive wrist movements can lead to strain injuries. If the yoga class involved a lot of poses that put pressure on the wrists, this could contribute to inflammation or irritation of the nerves.

3. Circulation Issues: Sometimes, positions that restrict blood flow can lead to temporary numbness. If your arms were in a position that restricted circulation, this could explain the numbness.

4. Cervical Radiculopathy: Although less likely given your description, issues in the cervical spine (neck) can also lead to numbness in the arms and fingers. If there is a herniated disc or other cervical spine issue, it could compress nerves that travel down to the arms.

Recommendations
1. Rest and Observation: Since you mentioned that the numbness does not interfere with your daily activities and there is no pain, it may be beneficial to rest and observe your symptoms. Many cases of nerve compression resolve on their own with time.

2. Modify Yoga Practice: Consider modifying your yoga practice to avoid positions that put excessive strain on your wrists. You might want to discuss your symptoms with your yoga instructor, who can help you adjust your poses to prevent further irritation.

3. Gentle Stretching and Strengthening: Engaging in gentle wrist and hand stretches can help alleviate any tightness that may be contributing to the numbness. Strengthening exercises for the wrist and forearm can also be beneficial.

4. Consult a Healthcare Professional: If the numbness persists beyond a week or worsens, it would be wise to consult a healthcare professional. They may perform a physical examination and possibly imaging studies to rule out any underlying conditions.

5. Ergonomic Adjustments: If you engage in activities that require prolonged wrist use outside of yoga (like typing or using a smartphone), ensure that your workspace is ergonomically friendly. This can help reduce strain on your wrists and prevent further issues.

Conclusion
Persistent numbness in the thumb and index finger after yoga can often be attributed to nerve compression or strain. While it’s reassuring that you’re not experiencing pain or functional limitations, it’s essential to monitor your symptoms. If they persist or worsen, seeking medical advice is crucial to rule out any serious underlying conditions. In the meantime, modifying your yoga practice and incorporating gentle stretching can help alleviate your symptoms.
