The sensation of discomfort during urination?

Hello, Doctor.
This morning, I started to feel something unusual when urinating.
When the urine touches my clitoris, I experience a sour sensation; it's not pain, but it's very uncomfortable.
Additionally, I feel a sour, heavy sensation in my lower abdomen.
However, this doesn't happen every time I urinate today; sometimes, I feel normal at the beginning of urination, but towards the end, I experience this sensation, and I have also been feeling very frequent urges to urinate.
P.S.: The day before, I engaged in clitoral stimulation.
Could this possibly be a urinary tract infection or another issue? Thank you.

Miss He, 20~29 year old female. Ask Date: 2022/12/01

Dr. Sun Hongshun reply Urology

Hello Ms.
He: If there are white blood cells in the urine test, it indicates inflammation of the bladder (urinary tract).
If there are none, I recommend undergoing a pelvic examination (either gynecology or urology is fine).
It could also be due to localized redness, swelling, or pain.
However, since you mentioned experiencing a dull, aching sensation in your lower abdomen while urinating, it is advisable to seek medical evaluation for peace of mind.
Wishing you a speedy recovery.

Hello! Thank you for reaching out with your concerns regarding discomfort during urination. Based on your description, it sounds like you are experiencing a combination of symptoms that could be related to several potential causes, including urinary tract infections (UTIs), irritation from recent sexual activity, or even pelvic floor dysfunction.

Firstly, the sensation of discomfort or a burning feeling when urinating, particularly if it is accompanied by a frequent urge to urinate, is often indicative of a urinary tract infection. UTIs are common and can occur when bacteria enter the urinary tract, leading to inflammation and irritation. Symptoms typically include a burning sensation during urination, increased frequency of urination, and sometimes lower abdominal discomfort. Given that you mentioned engaging in stimulating activities the day prior, it is possible that this could have contributed to irritation or even introduced bacteria into the urinary tract.

Another possibility is that the discomfort you are experiencing could be related to pelvic floor dysfunction. This condition occurs when the muscles of the pelvic floor are not functioning properly, which can lead to a variety of symptoms, including pain during urination, a feeling of heaviness in the lower abdomen, and difficulty fully emptying the bladder. Stress or tension in these muscles can also exacerbate these symptoms.

Additionally, if you have recently changed your hygiene products or if there has been any irritation from soaps or other substances, this could also lead to discomfort during urination. It’s essential to consider any recent changes in your routine that might have contributed to this sensation.

To address your symptoms, here are some recommendations:
1. Hydration: Increase your fluid intake, particularly water. Staying well-hydrated can help dilute your urine and may alleviate some discomfort.

2. Avoid Irritants: Stay away from caffeine, alcohol, spicy foods, and acidic foods, as these can irritate the bladder and worsen symptoms.

3. Hygiene Practices: Ensure that you are practicing good hygiene, especially after sexual activity. Urinating after intercourse can help flush out any bacteria that may have been introduced.

4. Warm Compress: Applying a warm compress to your lower abdomen may help relieve discomfort.

5. Consult a Healthcare Provider: Given that your symptoms have persisted and are causing you discomfort, it is advisable to consult a healthcare provider. They may recommend a urinalysis to check for signs of infection or other underlying issues. If a UTI is diagnosed, antibiotics may be prescribed.

6. Pelvic Floor Therapy: If pelvic floor dysfunction is suspected, a referral to a physical therapist specializing in pelvic health may be beneficial. They can provide exercises and techniques to help relax and strengthen the pelvic floor muscles.

7. Monitor Symptoms: Keep track of your symptoms, including when they occur and any associated activities. This information can be helpful for your healthcare provider in determining the cause of your discomfort.

In conclusion, while your symptoms could be indicative of a UTI or pelvic floor dysfunction, it is essential to seek professional medical advice for an accurate diagnosis and appropriate treatment. Do not hesitate to reach out to a healthcare provider, especially if your symptoms worsen or do not improve with self-care measures. Your health and comfort are paramount, and addressing these concerns early can lead to better outcomes.
