Blood-Streaked Nasal Discharge: Causes and Concerns - Otolaryngology

I have a question I would like to ask..?

I have been experiencing a runny nose frequently since middle school, especially during weather changes or cold fronts, so blowing my nose has become a common occurrence for me.
The doctor diagnosed me with nasal allergies.
Last October, when I started basic training, the colder morning weather and living in close quarters with many others led to several of my fellow trainees starting to cough and catch colds, and I also began to have a runny nose and cough.
This condition persisted for several days, and due to frequently blowing my nose, I noticed some blood streaks in my nasal discharge (at the time, I suspected it was nasal mucosal bleeding).
After a few more days, I found that my nasal discharge contained dark red blood, and at its worst, the mucus I expelled was visibly covered in dark red blood (I had never had a nosebleed before).
I was quite anxious, and some of my fellow trainees suggested that I shouldn't blow my nose too hard.
I changed my nose-blowing habits, and sure enough, after about two days, this phenomenon disappeared, although I still had a runny nose.
Later, I visited a military hospital, where the doctor examined my chest X-ray and noted signs of bronchitis, recommending that I be hospitalized for three to four days for rest.
After my hospitalization, my condition improved significantly.
Once basic training was over, I went to an ENT clinic near my home, where the doctor used an endoscope to examine my nasal cavity and diagnosed me with sinusitis.
He suggested that my cough might be due to post-nasal drip and recommended that I wear a mask when I wake up early.
After taking medication for about one to two weeks, my nose returned to normal.
Now, a year later, my nose is still relatively normal! I suddenly recalled this situation and would like to ask the doctor: Is my understanding correct that the dark red blood mixed in with my nasal discharge was due to nasal mucosal bleeding that accumulated in the sinuses and mixed with the mucus, and that the dark red color of the blood may have been due to it staying in the sinuses for a period of time? PS: Thank you, doctor, for patiently reading my question! Thank you!

Understanding blood-streaked nasal discharge can be quite concerning, especially when it occurs frequently or in unusual amounts. Based on your description, it seems that you have experienced a series of respiratory issues that have led to the presence of blood in your nasal discharge. Let’s break down the potential causes and implications of your symptoms.

Firstly, the presence of blood in nasal discharge, especially when it appears as streaks or mixed with mucus, is often due to the rupture of small blood vessels in the nasal mucosa. This can happen for several reasons, including:
1. Nasal Trauma: Frequent nose blowing, especially when done forcefully, can lead to micro-tears in the delicate nasal lining. This is particularly common during cold weather or when experiencing allergies, as you mentioned. The blood you observed could indeed be from these small ruptures.

2. Dry Air: Cold weather often comes with lower humidity levels, which can dry out the nasal passages. This dryness can lead to crusting and subsequent bleeding when the crusts are disturbed by blowing your nose.

3. Infection: Conditions like sinusitis or bronchitis, which you were diagnosed with, can cause inflammation and swelling in the nasal passages. This inflammation can make the blood vessels more susceptible to breaking, especially if you are coughing or blowing your nose frequently.

4. Underlying Conditions: While less common, conditions such as hypertension or clotting disorders can also lead to increased bleeding tendencies. However, given your history of allergies and respiratory infections, these are less likely to be the primary cause in your case.

Regarding your observation about the dark red blood, it is plausible that the blood could appear darker if it has been stagnant in the nasal passages or sinuses for a period of time. Fresh blood is typically bright red, while older blood that has been exposed to air can turn darker due to oxidation. This aligns with your understanding that the blood may have pooled in the sinuses before being expelled with nasal mucus.

Your experience of changing your nose-blowing technique and noticing an improvement is a common and effective strategy. Gentle blowing can help minimize trauma to the nasal mucosa, allowing any small injuries to heal. Additionally, using a humidifier in dry environments, staying hydrated, and using saline nasal sprays can help maintain moisture in the nasal passages, reducing the likelihood of bleeding.

It’s also worth noting that if you experience recurrent episodes of blood-streaked nasal discharge, or if the bleeding is significant, it would be wise to consult with an ENT specialist. They can perform a thorough examination, possibly including nasal endoscopy, to rule out any structural issues or other underlying conditions that might be contributing to your symptoms.

In summary, your understanding of the situation is largely correct. The dark red blood in your nasal discharge likely resulted from minor trauma to the nasal mucosa, compounded by the effects of dry air and respiratory infections. Maintaining gentle nasal hygiene and monitoring your symptoms will be key in managing this condition. If symptoms persist or worsen, seeking further medical evaluation is advisable. Thank you for your thoughtful question, and I hope this information helps clarify your concerns!
