Bleeding 7-14 days after the end of menstruation?
Hello, I am 26 years old and have a steady partner.
My menstrual cycle was very regular until about six months ago.
Now, I usually experience bleeding (or spotting) about 7 to 14 days after my period ends (typically around 14 days).
The amount is not significant, and I usually notice it after using the restroom.
It typically only occurs at the time of discovery and then stops.
My physical condition includes abdominal pain and lower back pain before my period, and when my period arrives, I experience severe abdominal pain and my legs often feel numb (or sore).
I usually take one acetaminophen for the menstrual pain.
I am unsure about what this situation might indicate and am concerned whether it could be endometriosis or if it might affect my fertility (infertility).
I also wonder if I need to undergo a Pap smear (I have not had any examinations yet).
Thank you.

Director Wu Zongcheng of the Obstetrics and Gynecology Department at Tainan Hospital, Department of Health, responded: (1) Spotting a few days after the end of menstruation is related to hormonal secretion abnormalities, commonly referred to as "ovulatory bleeding." This occurs due to insufficient progesterone secretion, leading to the shedding of the endometrial lining and resulting in spotting.
(2) For menstrual pain, please consult an obstetrician-gynecologist to determine if there is endometriosis or a chocolate cyst.
(3) Women over 30 should have a Pap smear annually.

Post-menstrual bleeding, or bleeding that occurs after the menstrual period has ended, can be concerning for many women. It is important to understand the potential causes and implications of this symptom, especially if it is a new occurrence or has changed in frequency or intensity.
In your case, you mentioned that you are 26 years old and have a regular partner, with previously normal menstrual cycles. However, over the past six months, you have experienced bleeding 7 to 14 days after your period, which is unusual for you. The bleeding is described as light and often consists of spotting or blood-tinged discharge. You also noted that you experience significant abdominal pain and discomfort during your menstrual cycle, which may suggest underlying issues.
There are several potential causes for post-menstrual bleeding, including:
1. Hormonal Imbalances: Fluctuations in hormone levels can lead to irregular bleeding. Conditions such as polycystic ovary syndrome (PCOS) or thyroid disorders can disrupt the normal menstrual cycle and result in unexpected bleeding.
2. Ovulation Bleeding: Some women experience light spotting during ovulation, which typically occurs about 14 days before the next period. This can be mistaken for post-menstrual bleeding.
3. Uterine Polyps or Fibroids: These benign growths in the uterus can cause irregular bleeding. While they are not cancerous, they can lead to discomfort and changes in menstrual patterns.
4. Endometriosis: This condition occurs when tissue similar to the lining of the uterus grows outside of it, often causing severe pain and irregular bleeding. It can also impact fertility.
5. Infections: Pelvic inflammatory disease (PID) or other infections can lead to abnormal bleeding and should be evaluated by a healthcare provider.
6. Cervical or Uterine Issues: Conditions affecting the cervix or uterus, such as cervical ectropion or endometrial hyperplasia, can also cause irregular bleeding.
Given your concerns about conditions like endometriosis or potential impacts on fertility, it is advisable to consult with a healthcare provider for a thorough evaluation. A pelvic exam, ultrasound, and possibly a Pap smear can help rule out serious conditions and provide clarity on the cause of your symptoms.
Additionally, if you are experiencing significant pain during your menstrual cycle, it is important to address this with your doctor. Painful periods (dysmenorrhea) can be a sign of underlying conditions such as endometriosis or fibroids, which may require specific treatment.
In summary, while post-menstrual bleeding can be caused by various factors, it is essential to seek medical advice to determine the underlying cause and appropriate management. Regular check-ups and open communication with your healthcare provider can help ensure your reproductive health is monitored and maintained. If you have not yet had a Pap smear, it is also a good idea to discuss this with your doctor, as it is an important part of preventive health care for women.
