The right testicle is slightly hard?

Hello, doctor.
When I touch my right testicle, it feels significantly larger than the left one, and the surface cannot be easily compressed.
It doesn't feel elastic as described online; rather, it feels tense and firm.
The left testicle can be slightly compressed, but it doesn't feel elastic either, and after pressing down a bit, its firmness feels similar to that of the right testicle.
About a month and a half ago, I visited a hospital and consulted a doctor, who said that this was a normal phenomenon as long as the testicles feel smooth without any particularly prominent lumps.

1.
Is it true that if the testicles feel relatively firm but are similar in hardness on both sides and there are no noticeable lumps, it can be considered normal?
2.
When the weather gets colder or during a shower, I notice that the left testicle feels similar in size and firmness to the right one, while the right one does not change much.
Is this normal? Thank you for taking the time to respond, doctor.

More Info

When it comes to testicular health, it's essential to understand that any noticeable changes in size, shape, or consistency can warrant further investigation. Based on your description, it seems you have some concerns regarding the hardness and size of your right testicle compared to the left. Here’s a detailed response to your questions, along with some additional insights into testicular health.

1. Normal Variations in Testicular Hardness: The hardness of the testicles can vary from person to person. It is not uncommon for one testicle to feel firmer than the other. However, the key factors to consider are whether there are any significant changes in size, shape, or if there are any lumps or masses that can be felt. If your doctor has examined you and noted that the testicles feel normal and there are no concerning lumps, it is likely that what you are experiencing is within the range of normal anatomical variation. However, if you feel that the hardness of the right testicle is significantly different from the left, or if you notice any other symptoms (such as pain, swelling, or changes in skin texture), it is advisable to seek a second opinion or further evaluation.

2. Temperature Effects on Testicular Size: It is normal for testicles to change size and position in response to temperature. The cremaster muscle, which surrounds the testicles, contracts in cold temperatures to pull the testicles closer to the body for warmth and relaxes in warmer conditions, allowing them to hang lower. This can create the sensation that the left testicle is becoming firmer and larger when exposed to heat (like during a warm shower) while the right one remains unchanged. This is a normal physiological response and not typically a cause for concern.

Additional Considerations
- Testicular Health Monitoring: Regular self-examinations are crucial for maintaining testicular health. You should perform these checks monthly, looking for any changes in size, shape, or consistency. If you notice any lumps, swelling, or persistent discomfort, it is essential to consult a healthcare provider.

- Potential Conditions: While variations in testicular firmness can be normal, certain conditions can cause changes in testicular consistency. These include:
- Epididymitis: Inflammation of the epididymis, often due to infection, which can cause swelling and tenderness.

- Hydrocele: A fluid-filled sac around the testicle that can cause swelling.

- Varicocele: Enlarged veins within the scrotum that can lead to a feeling of heaviness or discomfort.

- Testicular Tumors: While less common, any hard mass or significant change in texture should be evaluated to rule out malignancy.

- When to Seek Medical Advice: If you experience any of the following, you should seek medical attention:
- Persistent pain or discomfort in the testicles.

- Noticeable swelling or changes in size that are not related to temperature.

- Any lumps or masses that feel different from the surrounding tissue.

- Changes in skin texture or color of the scrotum.

In conclusion, while some variations in testicular hardness and size can be normal, it is essential to remain vigilant and proactive about your health. If you have any doubts or concerns, don’t hesitate to consult a healthcare professional for a thorough examination and peace of mind. Regular check-ups and self-examinations are key to ensuring your reproductive health remains optimal.
