Ear problems

Hello doctor, I often have earwax that does not fall out on its own, and after accumulating for a while, I clean my ears and find a hard mass in the ear canal that is flesh-colored and slightly white, about 0.5 cm in size.
It is not painful, and I can barely touch it with my finger; it feels hard.
What could be the possible cause? Thank you.

A-Nan, 30~39 year old female. Ask Date: 2023/02/07

Dr. Zheng Jueyi reply Otolaryngology

If you are unable to determine the cause and are concerned or if the symptoms persist, it is recommended to visit an otolaryngologist for examination and confirmation.

Earwax buildup, also known as cerumen impaction, is a common issue that many people experience. Earwax, or cerumen, is a natural substance produced by glands in the ear canal. It serves several important functions, including protecting the ear from dust, debris, and microorganisms, as well as lubricating the ear canal to prevent dryness and itching. However, when earwax accumulates excessively, it can lead to various problems, including discomfort, hearing loss, and even infections.

Causes of Earwax Buildup
1. Overproduction of Earwax: Some individuals naturally produce more earwax than others. This can be influenced by genetics, age, and even certain skin conditions.

2. Ear Canal Shape: The anatomy of the ear canal can affect how earwax is expelled. People with narrow or curved ear canals may find that earwax does not exit the ear as easily.

3. Use of Earplugs or Hearing Aids: Regular use of earplugs or hearing aids can push earwax deeper into the ear canal, leading to buildup.

4. Improper Cleaning Techniques: Using cotton swabs or other objects to clean the ears can inadvertently push earwax further into the canal, causing impaction.

5. Environmental Factors: Exposure to dust, dirt, and other environmental pollutants can increase earwax production as the body attempts to protect the ear.

Symptoms of Earwax Buildup
Individuals with earwax buildup may experience a variety of symptoms, including:
- Earache or discomfort
- Hearing loss or muffled hearing
- Tinnitus (ringing in the ears)
- A feeling of fullness in the ear
- Itching or irritation in the ear canal
- Odor or discharge from the ear

Solutions for Earwax Buildup
1. Avoid Cotton Swabs: It is important to avoid using cotton swabs or other objects to clean the ears, as this can push wax deeper and cause impaction.

2. Ear Drops: Over-the-counter ear drops can help soften earwax, making it easier for the body to expel it naturally. These drops often contain ingredients like hydrogen peroxide or mineral oil.

3. Irrigation: Some people may benefit from gentle irrigation of the ear canal using warm water or saline solution. This should be done carefully to avoid damaging the ear.

4. Professional Cleaning: If earwax buildup is severe or causing significant symptoms, it is advisable to seek professional help. An ear, nose, and throat (ENT) specialist can safely remove earwax using specialized instruments.

5. Regular Check-ups: For individuals prone to earwax buildup, regular check-ups with a healthcare provider can help monitor and manage the condition.

Regarding the Hard Lump in Your Ear Canal
The hard lump you described, which is approximately 0.5 cm in size and has a flesh-colored appearance, could potentially be impacted earwax, a sebaceous cyst, or another benign growth. It is important to avoid attempting to remove it yourself, as this could lead to injury or infection.

When to Seek Medical Attention
If you experience any of the following, it is important to consult a healthcare professional:
- Persistent pain or discomfort in the ear
- Significant hearing loss
- Discharge from the ear that is unusual or has an odor
- Symptoms of infection, such as fever or swelling
In conclusion, earwax buildup is a common issue that can lead to discomfort and other ear-related problems. Understanding the causes and solutions can help you manage this condition effectively. If you have concerns about a hard lump in your ear canal or if symptoms persist, it is best to seek evaluation from a qualified healthcare provider.
