Left lower abdominal discomfort and bloating, accompanied by frequent belching, flatulence, and the presence of intestinal fluid in the stool?
Hello Doctor: I would like to ask about a concern I've been experiencing in recent months.
After dinner, I often feel a sense of heaviness and bloating in my lower left abdomen.
Occasionally, I also feel a dull pain above my navel.
I have a persistent sensation of gas in my lower left abdomen, leading to frequent burping and flatulence.
During the day, everything seems normal; my bowel movements are regular and typically occur at the same time, although they are mostly soft stools without diarrhea.
After each bowel movement, I have a habit of washing with warm water, and I notice that there is a slippery intestinal fluid in my anal area.
Is this normal?
Awei, 40~49 year old female. Ask Date: 2023/02/26
Dr. Chen Shidian reply Gastroenterology and Hepatology
Still within normal limits, can continue to monitor.

Left lower abdominal discomfort can arise from a variety of causes, and understanding these can help in determining the appropriate course of action. Based on your description, it seems that you are experiencing a combination of symptoms including bloating, occasional pain, and gas, particularly after meals.
Firstly, it is important to consider the gastrointestinal tract as a primary source of your discomfort. Conditions such as irritable bowel syndrome (IBS), which is characterized by symptoms like bloating, gas, and changes in bowel habits, could be a potential cause. IBS can often lead to discomfort after eating, especially if the meal is rich in fats or fiber, which can be harder to digest. The presence of gas and the need to burp or pass gas can also be indicative of dietary intolerances, such as lactose intolerance or sensitivity to certain carbohydrates (FODMAPs).
Another possibility is that your symptoms could be related to the digestive process itself. After eating, the body directs blood flow to the digestive organs, which can sometimes lead to sensations of fullness or discomfort in the abdomen. If you are experiencing a feeling of pressure or bloating, it may be due to the accumulation of gas in the intestines, which can occur for various reasons, including swallowing air while eating or drinking, or the fermentation of undigested food by gut bacteria.
Regarding the presence of a slippery anal discharge after bowel movements, it is not uncommon for the body to produce mucus in the intestines, which can sometimes be expelled during bowel movements. However, if this is accompanied by other symptoms such as blood in the stool, significant changes in bowel habits, or persistent pain, it would be advisable to seek further evaluation.
Given your symptoms, it would be prudent to consult with a gastroenterologist. They can perform a thorough evaluation, which may include a physical examination, a review of your dietary habits, and possibly imaging studies or endoscopic procedures if necessary. Keeping a food diary to track what you eat and any symptoms that arise can also be helpful in identifying potential triggers.
In the meantime, consider making some dietary adjustments. Eating smaller, more frequent meals rather than larger ones can help reduce bloating. Additionally, avoiding carbonated beverages, high-fat foods, and certain high-fiber foods that are known to cause gas may provide some relief. Staying hydrated and incorporating gentle physical activity can also aid digestion.
In summary, while your symptoms may be related to common gastrointestinal issues such as IBS or dietary intolerances, a thorough evaluation by a healthcare professional is essential to rule out any serious underlying conditions. By addressing your symptoms and making lifestyle adjustments, you can work towards finding relief and improving your overall digestive health.
