Taking amoxicillin 500 mg?
Hello, I would like to ask if during my doctor's visit, I informed them that I am taking the oral contraceptive pill (Yaz), and later when they prescribed medication, they reassured me that it would not affect the efficacy of Yaz.
However, I just found out that I was prescribed amoxicillin 500mg.
Does this have any impact on the effectiveness of the oral contraceptive?

Dr. Zhang Kunmin reply Obstetrics and Gynecology
Hello, the following medications can interact with oral contraceptives:
1.
Antibiotics (such as Penicillin, Cephalosporins, Erythromycin, Sulfonamides, Bactrim, Chloramphenicol, Doxycycline) may reduce the effectiveness of oral contraceptives.
When taking these medications, alternative contraceptive methods should be employed, as they may decrease the gut flora that aids in absorption, thereby lowering the efficacy of the contraceptive.
2.
Sedatives (Barbiturates) can increase the metabolism of oral contraceptives, reducing their effectiveness.
3.
Antiepileptic drugs (such as Primidone, Phenytoin, Carbamazepine) can also increase the metabolism of oral contraceptives, leading to decreased effectiveness.
4.
Antituberculosis medications (Rifampicin, Isoniazid).
5.
Antidiabetic medications (Rosiglitazone).
6.
Acetaminophen and Ascorbic acid (Vitamin C) may inhibit the metabolism of estrogen, potentially increasing its concentration in the body and leading to a higher incidence of side effects.
7.
Oral contraceptives can affect the action of anticoagulants.
8.
Smoking can increase the metabolism of oral contraceptives, reducing their effectiveness.
9.
Long-term consumption of certain teas or herbs may also have additive or subtractive effects with oral contraceptives.
Additionally, any medication should be taken with water and not with tea, coffee, or other beverages, as this may affect the medication's efficacy.
Therefore, if you need to take any of the aforementioned medications while on oral contraceptives, it is advisable to use additional contraceptive measures for seven days.
Thank you! Wishing you safety and health, Dr.
Zhang.

More Info
Amoxicillin is a commonly prescribed antibiotic that belongs to the penicillin class. It is often used to treat various bacterial infections. When it comes to its interaction with hormonal contraceptives, such as birth control pills, there has been considerable debate and research over the years.
The concern regarding antibiotics and birth control pills primarily stems from the potential for antibiotics to affect the gut flora, which can, in turn, influence the absorption of the hormones in the contraceptive pills. However, the majority of studies have shown that most antibiotics, including amoxicillin, do not significantly reduce the effectiveness of birth control pills. This is particularly true for amoxicillin, which is not known to interfere with the metabolism of estrogen or progestin, the hormones commonly found in oral contraceptives.
The confusion often arises from the fact that some antibiotics, particularly rifampicin and rifabutin, have been shown to lower the effectiveness of hormonal contraceptives. These antibiotics induce liver enzymes that metabolize the hormones more quickly, thereby reducing their levels in the bloodstream. However, amoxicillin does not have this effect, and thus, it is generally considered safe to use while on birth control pills without the need for additional contraceptive measures.
That said, it is always prudent to consult with your healthcare provider regarding any concerns about drug interactions. If you are taking amoxicillin and are worried about the effectiveness of your birth control, it is advisable to use a backup method of contraception, such as condoms, during the course of antibiotic treatment and for a short period afterward, just to be safe.
In your case, since your doctor has indicated that amoxicillin does not affect the effectiveness of your birth control pill (Yueh-Ji), and considering the general consensus in the medical community, you should be able to continue taking your birth control pill without significant concern. However, if you experience any unusual symptoms or have further questions, do not hesitate to reach out to your healthcare provider for personalized advice.
In summary, while there are some antibiotics that can affect the efficacy of birth control pills, amoxicillin is not one of them. You can continue taking your birth control as prescribed, but always keep an open line of communication with your healthcare provider regarding any medications you are taking.
