Genital skin abrasion?
In the past few months, I have noticed that a couple of days before my menstrual period, there are small abrasions near the urethral opening (very tiny wounds that are hard to see).
I usually have no symptoms, but these small wounds appear just before my period, causing a stinging sensation when I urinate.
Is there any ointment I can apply to improve this condition? (The pain is only from the abrasions, and there is no itching.) Additionally, I believe the likelihood of a urinary tract infection is low, as I have been very careful to drink plenty of water and urinate regularly since I had one in high school.
Even when I am not menstruating, my urination is normal and painless, and I drink more than 2000cc of water daily.

More Info
Understanding skin irritation near the urethra, especially in relation to the menstrual cycle, can be quite concerning. The symptoms you describe—small cuts or abrasions near the urethra that occur just before your period—could be attributed to several factors, including hormonal changes, skin sensitivity, or even friction.
Hormonal Changes and Skin Sensitivity
In the days leading up to menstruation, your body undergoes significant hormonal fluctuations, particularly in estrogen and progesterone levels. These hormonal changes can lead to increased sensitivity of the skin, which may make it more prone to irritation or injury. The skin in the genital area is particularly delicate, and any increase in sensitivity can result in minor abrasions, especially if there is friction from clothing or activities.
Friction and Moisture
Another potential cause of the irritation could be friction. If you are wearing tight clothing or if there is increased moisture in the area (due to sweat or other factors), this can lead to skin breakdown. The genital area is often subject to friction, and during the menstrual cycle, the presence of menstrual products can exacerbate this issue. It’s essential to ensure that you are wearing breathable, cotton underwear and that you change menstrual products regularly to minimize irritation.
Recommendations for Treatment
1. Topical Treatments: For the small cuts or abrasions, applying a gentle, over-the-counter topical antibiotic ointment (like Neosporin) can help prevent infection and promote healing. If you prefer something more natural, a soothing aloe vera gel may also provide relief and hydration to the affected area.
2. Avoid Irritants: Be cautious with soaps, lotions, or other products that may contain fragrances or harsh chemicals. Opt for gentle, hypoallergenic products designed for sensitive skin.
3. Hydration and Hygiene: Continue to maintain good hydration, as you are already doing. Drinking plenty of water helps to dilute urine, which can reduce irritation during urination. Additionally, ensure that you are practicing good hygiene, especially during your menstrual cycle.
4. Consult a Healthcare Provider: If the irritation persists or worsens, it would be wise to consult a healthcare provider. They can evaluate for any underlying conditions, such as vulvodynia or other dermatological issues, and provide tailored treatment options.
Urinary Tract Infection (UTI) Consideration
While you mentioned that you believe the likelihood of a urinary tract infection (UTI) is low, it’s essential to remain vigilant. Symptoms of a UTI can sometimes be subtle, and if you experience any changes in urination patterns, increased pain, or other unusual symptoms, it would be prudent to seek medical advice.
Conclusion
In summary, the skin irritation you are experiencing near the urethra before menstruation could be related to hormonal changes, friction, or sensitivity. Using a gentle topical treatment, avoiding irritants, and maintaining good hygiene practices can help alleviate the symptoms. If the problem persists, consulting a healthcare provider is the best course of action to rule out any underlying conditions and receive appropriate care. Remember, your health and comfort are paramount, and addressing these concerns early can lead to better outcomes.
