Diarrhea with bleeding
Hello doctor, every time I have diarrhea, it starts off normal, but after several episodes, I end up passing mucus with blood.
Is this due to damage to the intestinal mucosa, an issue with the small intestine, or could it be another condition? (I had a colonoscopy at the end of February this year, which showed no polyps or tumors, but I do have internal hemorrhoids.)

Dr. Chen Shidian reply Gastroenterology and Hepatology
If there is only temporary mucosal damage, observation and treatment for diarrhea can continue.
If there is also weight loss, further medical evaluation should be sought to determine the cause.

Blood in diarrhea can be a concerning symptom, and understanding its causes is crucial for proper diagnosis and treatment. Given your description, it seems that you experience normal stools initially, followed by diarrhea that eventually contains mucus and blood. This pattern can indicate several potential issues within the gastrointestinal tract.
Firstly, the presence of blood in your stool, especially if it is mixed with mucus, could suggest inflammation or irritation of the intestinal lining. Conditions such as infectious gastroenteritis, inflammatory bowel disease (IBD), or irritable bowel syndrome (IBS) can lead to such symptoms. Infections caused by bacteria, viruses, or parasites can cause inflammation, leading to bloody diarrhea. Common bacterial infections include those caused by Salmonella, Shigella, or Escherichia coli (E. coli). These infections often present with diarrhea, abdominal cramps, and sometimes fever.
Another possibility is ulcerative colitis, a type of IBD that primarily affects the colon and rectum. It can cause symptoms such as bloody diarrhea, abdominal pain, and urgency to defecate. Since you mentioned having a colonoscopy in February that showed no polyps or tumors, it may be worth considering whether any inflammatory conditions were overlooked, as some forms of IBD can be subtle in their presentation.
Additionally, the presence of hemorrhoids (internal or external) can also lead to blood in the stool, especially if there is straining during bowel movements. However, since you mentioned that the blood is mixed with mucus, this points more towards a possible mucosal injury or inflammation rather than just hemorrhoids.
Given that you have had a recent colonoscopy, it is less likely that a significant structural issue like a tumor is present, but it is still essential to follow up with your healthcare provider. They may recommend further testing, such as stool cultures, imaging studies, or even repeat endoscopy, to investigate the cause of your symptoms more thoroughly.
In summary, the presence of blood in your diarrhea can stem from various causes, including infections, inflammatory bowel diseases, or even hemorrhoids. It is essential to monitor your symptoms closely and consult with your healthcare provider for a comprehensive evaluation and appropriate management. If you experience additional symptoms such as severe abdominal pain, fever, or dehydration, seek medical attention promptly.
