Torn foreskin with bleeding?


Hello, doctor.
I have not engaged in any high-risk sexual behavior, but I do have phimosis and always use condoms during sexual intercourse.
I previously had symptoms of balanitis, which have resolved.
However, this time the sensation is different; recently, the tip of my foreskin has been tearing and bleeding during sexual activity.
It does not hurt or itch otherwise, and it heals naturally within two days, appearing smooth and tender afterward.
However, if I engage in sexual activity again, the same area tears and bleeds.
Pulling the foreskin back over the glans also causes severe pain due to erection.
I have had blood tests for diabetes, and there are no signs of prediabetes.
I am curious if this could be eczema, as the repeated tearing and bleeding occurs in the same spot.
Is this more indicative of inflammation or eczema? The affected area is relatively small and does not cause pain or itching, which is quite distressing.
(I have also seen a doctor who prescribed medication for eczema, but after applying it and waiting a month before engaging in sexual activity, I still experienced tearing and bleeding.)
