I have been suffering for a long time?
I have been experiencing poor mental health for about 4.5 years.
I feel particularly uncomfortable when I wake up in the morning and am almost lethargic throughout the entire morning.
I have undergone blood and urine tests, but it’s strange that when I suspected I had anemia and got tested, the doctor said I have thalassemia.
However, thalassemia typically has a 50% carrier rate, a 25% severe form, and a 25% non-hereditary form.
None of my family members have this condition, so I wonder if I might have been misdiagnosed.
Additionally, I was told that my liver function is slightly impaired, but still acceptable.
I have seen many doctors, and none have been able to determine the cause of my issues.
I am currently living in a state of distress and have not been able to find relief.
Please help me.
Thank you.

Dr. Huang Jianling reply Family Medicine
Hello Wei,
Your concern mainly revolves around the uncertainty of whether you have thalassemia (commonly referred to as Mediterranean anemia) and liver function abnormalities, compounded by symptoms of morning fatigue.
It is advisable to visit a family medicine specialist for further diagnostic confirmation and to determine if treatment is necessary.
Below are responses regarding anemia and liver function abnormalities:
1.
Anemia: If the red blood cell volume is too small, additional tests for serum iron and screening for alpha and beta thalassemia can help differentiate the condition.
2.
Liver Function Abnormalities: The most common cause is fatty liver disease, followed by hepatitis B and C, alcohol consumption, and chemical drug exposure.
Therefore, if liver function abnormalities are present, it is prudent to first check for hepatitis B and C, followed by an abdominal ultrasound to assess for fatty liver, and finally rule out chemical drug exposure or autoimmune diseases.
Sincerely, Dr.

More Info
It sounds like you are going through a very challenging time with your health, and I want to acknowledge how difficult it can be to navigate unexplained health issues. Based on your description, it seems that you have been experiencing persistent fatigue and discomfort, particularly in the mornings, along with a diagnosis of Mediterranean anemia and some liver function concerns.
Firstly, it’s important to understand that Mediterranean anemia, also known as thalassemia, is a genetic blood disorder that affects hemoglobin production. While it is true that this condition can be inherited, it is also possible for individuals to be carriers without showing significant symptoms or having affected family members. If your family does not have a history of this condition, it might be worth discussing with a hematologist to confirm the diagnosis and explore whether further genetic testing could provide clarity.
In addition to the anemia, you mentioned experiencing fatigue and discomfort, which can be linked to various factors. Chronic fatigue can stem from a multitude of causes, including sleep disorders, nutritional deficiencies, stress, and underlying medical conditions. Given your history of liver function issues, it would be prudent to ensure that your liver health is being monitored closely, as liver dysfunction can contribute to feelings of fatigue and malaise.
Here are some steps you can take to address your health concerns:
1. Comprehensive Evaluation: If you haven’t already, consider seeking a comprehensive evaluation from a specialist, such as a hematologist for your anemia and a gastroenterologist for your liver function. They can provide targeted tests and assessments to better understand your condition.
2. Nutritional Assessment: A dietitian can help you evaluate your diet to ensure you are getting adequate nutrients, particularly iron, vitamin B12, and folate, which are essential for red blood cell production. Sometimes, even with a healthy diet, deficiencies can occur due to absorption issues.
3. Sleep Hygiene: Since you mentioned waking up feeling unwell, it may be beneficial to assess your sleep quality. Poor sleep can significantly impact your energy levels. Consider establishing a regular sleep routine, creating a comfortable sleep environment, and possibly discussing sleep disorders with a healthcare provider.
4. Mental Health Support: Chronic health issues can take a toll on mental well-being. If you are feeling overwhelmed, anxious, or depressed, it might be helpful to speak with a mental health professional. Therapy can provide coping strategies and emotional support as you navigate your health challenges.
5. Regular Monitoring: Keep track of your symptoms, dietary habits, and any changes in your health. This information can be invaluable for your healthcare providers in determining the best course of action.
6. Lifestyle Modifications: Incorporating regular, gentle exercise can help improve your overall energy levels and mood. Activities such as walking, yoga, or swimming can be beneficial. Additionally, practicing stress-reduction techniques like mindfulness or meditation can help manage anxiety and improve your overall sense of well-being.
7. Second Opinions: If you feel that your concerns are not being adequately addressed, don’t hesitate to seek second opinions from other healthcare providers. Sometimes, a fresh perspective can lead to new insights and solutions.
In conclusion, while it can be frustrating and disheartening to deal with unexplained health issues, there are steps you can take to seek clarity and improve your situation. It’s important to advocate for yourself and continue pursuing answers until you find a healthcare team that can help you navigate your health challenges effectively. Remember, you are not alone in this journey, and there are resources and support available to help you through it.
