Blood after bowel movement?
Doctor, every time I have a bowel movement, as long as it's not diarrhea (meaning it's firm and formed), I notice blood when I wipe.
This has been happening for a while.
What could be causing this?

Hello! When stool is expelled from the rectum, the anal sphincter muscles contract, and sometimes due to the stool being hard, excessive straining can occur, which may lead to anal fissures and bleeding.
Additionally, if there are hemorrhoids (internal or external), this can also cause bleeding; however, bleeding from hemorrhoids is usually more significant.
Furthermore, if there are lesions in the rectum or colon, such as polyps, inflammation, or tumors, bleeding may also occur.
Therefore, to obtain an accurate diagnosis, it is best to undergo a rectal or sigmoidoscopic examination to avoid delaying treatment and causing serious consequences.
Sincerely, Dr.

Blood in the stool, especially when it appears after bowel movements, can be concerning and warrants a thorough evaluation. The presence of blood can be attributed to several causes, and understanding these can help in determining the appropriate course of action.
One of the most common causes of blood after bowel movements is hemorrhoids. Hemorrhoids are swollen veins in the lower rectum or anus that can become inflamed or irritated, leading to bleeding, especially during bowel movements. This is often accompanied by discomfort or itching. If the stool is hard, it can cause straining during bowel movements, exacerbating the issue.
Another potential cause is anal fissures, which are small tears in the lining of the anus. These can occur due to passing hard stools and can result in sharp pain and bleeding during or after bowel movements.
Other causes may include diverticular disease, where small pouches form in the colon wall and can become inflamed or bleed, and gastrointestinal conditions such as inflammatory bowel disease (IBD), which includes Crohn's disease and ulcerative colitis. These conditions can lead to more significant bleeding and other symptoms like abdominal pain and diarrhea.
In some cases, more serious conditions such as colorectal polyps or cancer can also present with blood in the stool. While these are less common, they are important to rule out, especially if there are other risk factors present, such as a family history of colorectal cancer.
Given that you have been experiencing this symptom for a while, it is crucial to consult a healthcare professional for a proper diagnosis. They may recommend a physical examination, possibly including a digital rectal exam, and further investigations such as a colonoscopy, which allows for direct visualization of the colon and rectum. This procedure can help identify any abnormalities, such as hemorrhoids, fissures, or polyps, and can also allow for biopsies if necessary.
In terms of solutions, if hemorrhoids or fissures are diagnosed, treatment may include dietary changes to increase fiber intake, which can help soften stools and reduce straining. Over-the-counter treatments, such as topical creams or ointments, can also provide relief. In more severe cases, medical procedures may be necessary to remove hemorrhoids or repair fissures.
If the bleeding is due to a more serious condition, the treatment will depend on the specific diagnosis. This may involve medications, lifestyle changes, or surgical interventions.
In summary, while blood after bowel movements can often be attributed to benign conditions like hemorrhoids or fissures, it is essential to seek medical advice to rule out more serious issues. Early diagnosis and treatment can significantly improve outcomes and alleviate symptoms. If you notice any additional symptoms such as significant pain, changes in bowel habits, or weight loss, be sure to mention these to your healthcare provider, as they can provide important clues to your condition.
