Palpitations-related issues
Hello Dr.
Wu: Recently, I have been experiencing palpitations at times (a sudden increase in the force of my heartbeat, and I can hear my heartbeat a little; I'm not sure if this qualifies as palpitations).
This mainly occurs in the following situations: 1.
When I wake up from a nap while lying on my stomach (I'm not sure if this happens every time), my heartbeat suddenly exhibits the aforementioned symptoms, but after a while (I'm not certain how long, maybe just a few beats, and I haven't deliberately counted), it subsides.
2.
It occurs suddenly when I feel a bit more fatigued.
3.
Sometimes it happens for no apparent reason (less frequently).
My lifestyle: I tend to go to bed late and get less sleep.
I also exercise less (due to an immune-related condition, I experience back pain, so I don't exercise very often).
Other: I had mild anxiety recently, but my condition has improved significantly; however, I'm not sure if I have completely recovered.
I would like to ask you, is this palpitations? Is this situation related to my lifestyle? Do I need to undergo any examinations? Thank you for your response!

Dr. Wu Xueming reply Cardiology
Due to your young age and lack of chronic diseases, your risk of developing heart disease is low.
Many factors in daily life can contribute to this situation, such as emotions, stress, irregular sleep patterns, or excessive intake of caffeinated beverages.
It is advisable to adjust your lifestyle and engage in regular exercise.
However, if symptoms persist, it is still recommended to visit a hospital for an electrocardiogram (ECG) examination.

More Info
Palpitations, or the sensation of feeling your heart beating, can be a common experience and may occur for a variety of reasons. Based on your description, it sounds like you are experiencing palpitations, especially when you wake up from a nap or feel fatigued. This sensation can be alarming, but it is important to understand the potential causes, symptoms, and when to seek medical help.
Causes of Palpitations
1. Lifestyle Factors: Your mention of late nights and insufficient sleep can significantly contribute to palpitations. Lack of sleep can lead to increased stress and anxiety levels, which may trigger heart palpitations. Additionally, if you are not engaging in regular physical activity, this can also affect your cardiovascular health.
2. Anxiety and Stress: You noted that you have experienced mild anxiety in the past. Anxiety can lead to heightened awareness of bodily sensations, including heartbeats. When you are anxious, your body releases stress hormones like adrenaline, which can cause your heart to beat faster or more forcefully.
3. Fatigue: Feeling tired or fatigued can also lead to palpitations. When your body is exhausted, it may react by increasing heart rate to compensate for decreased energy levels.
4. Medical Conditions: You mentioned having an immune-related condition, which could potentially affect your overall health and contribute to feelings of fatigue or discomfort. Certain medical conditions, such as hyperthyroidism or anemia, can also lead to palpitations.
5. Stimulants: Consumption of caffeine, nicotine, or certain medications can lead to palpitations. If you consume these substances, consider reducing your intake to see if your symptoms improve.
Symptoms of Palpitations
Palpitations can manifest in various ways, including:
- A racing or pounding heart
- Irregular heartbeats
- A feeling of fluttering in the chest
- Awareness of your heartbeat, especially when resting or lying down
When to Seek Help
While palpitations are often benign, there are certain situations where you should seek medical attention:
- If palpitations are accompanied by chest pain, shortness of breath, dizziness, or fainting.
- If you experience palpitations that are new or have changed in frequency or intensity.
- If you have a history of heart disease or other significant medical conditions.
Recommendations
1. Consult a Healthcare Provider: Given your symptoms and history of anxiety, it would be wise to consult with a healthcare provider. They may recommend tests such as an electrocardiogram (ECG) or a Holter monitor to assess your heart's rhythm and rule out any underlying conditions.
2. Lifestyle Modifications: Consider making some lifestyle changes, such as improving your sleep hygiene, incorporating regular physical activity (as tolerated), and managing stress through relaxation techniques like meditation or deep breathing exercises.
3. Monitor Your Symptoms: Keep a diary of when your palpitations occur, their duration, and any associated symptoms. This information can be helpful for your healthcare provider in determining the cause.
4. Limit Stimulants: If you consume caffeine or other stimulants, try reducing or eliminating them to see if your symptoms improve.
In conclusion, while palpitations can be concerning, they are often related to lifestyle factors, anxiety, or fatigue. However, it is essential to consult with a healthcare provider to ensure that there are no underlying medical issues. By addressing lifestyle factors and seeking appropriate medical advice, you can better manage your symptoms and improve your overall well-being.
