Postoperative issues following circumcision for concealed phimosis?
Hello Doctor: In February of this year, my son (first grader) underwent circumcision due to concealed phimosis, as the foreskin could not be retracted.
After evaluation, the doctor recommended the procedure.
Two weeks post-surgery, we noticed a small area of the outer skin that had no sensation due to swelling from the surgery.
When we inquired during the follow-up visit, the doctor suggested that we should continue to observe, as it could be due to nerve involvement or the current swelling affecting sensation.
Now, nearly two months post-surgery, that area still lacks sensation.
Is this a common occurrence following circumcision? Could this be considered a medical error? I apologize for my concerns; although it is just a portion of the outer skin without sensation, the surgical outcome has indeed led to this issue, and as a mother, I feel very guilty about my child's condition.
I would appreciate your clarification on this matter.
Thank you.
🙏🏻

Hello: Personally, I have not encountered this issue.
If there is nerve damage or injury, clinically, it is unlikely to have a complete loss of sensation.
Small areas of skin will need to wait for natural recovery.
Wishing you good health.

Post-circumcision concerns, particularly regarding sensation loss in children, can understandably cause anxiety for parents. It’s important to address these concerns with a comprehensive understanding of the procedure and its potential outcomes.
Circumcision is a common surgical procedure performed on infants and children for various medical and cultural reasons. While it is generally considered safe, like any surgical procedure, it carries risks and potential complications. One of the concerns that can arise post-surgery is the loss of sensation in the glans or the surrounding skin, which may be due to several factors.
1. Nerve Damage: During circumcision, the surgeon removes the foreskin, which contains a significant number of nerve endings. If the procedure inadvertently affects the nerves in the area, it could lead to a temporary or, in rare cases, permanent loss of sensation. However, it is essential to note that the glans itself is highly sensitive, and many children regain normal sensation over time as the area heals.
2. Swelling and Healing: Post-operative swelling is common and can temporarily affect sensation. If the area is still swollen, it might obscure normal sensory feedback. As the swelling subsides and the healing process continues, sensation may gradually return.
3. Time Frame for Recovery: In your case, since it has been nearly two months since the surgery, it is reasonable to expect that any swelling should have significantly decreased. If the area remains numb, it is advisable to follow up with your child's surgeon or a pediatric urologist. They can assess the situation more thoroughly and determine whether further intervention is necessary.
4. Medical Oversight: Regarding your concern about whether this situation constitutes medical negligence, it is crucial to understand that while complications can occur, they do not automatically imply malpractice. Medical professionals are trained to inform patients about potential risks, and some degree of variation in outcomes is expected. If the procedure was performed according to standard medical practices and the risks were communicated, it may not be considered negligence.
5. Next Steps: It is essential to maintain open communication with your child's healthcare provider. If you have concerns about the lack of sensation, request a follow-up appointment for a thorough evaluation. The doctor may conduct a physical examination and possibly recommend further tests to assess nerve function and healing.
6. Emotional Support: As a parent, it is natural to feel guilt or concern about your child's well-being. It is essential to provide emotional support to your child during this time. Reassure them that healing takes time and that they are not alone in this experience.
In conclusion, while sensation loss after circumcision can occur, it is often temporary and related to swelling or nerve impact during the procedure. Continued monitoring and communication with healthcare professionals are vital to ensuring your child's recovery and addressing any ongoing concerns. If the situation does not improve, further evaluation may be warranted to explore potential underlying issues.
