Swelling at the injection site one month after receiving the COVID-19 vaccine?

Hello, I would like to ask about my situation.
I received my fifth dose of the COVID-19 vaccine, the BA4.5 Moderna next-generation vaccine, on March 10.
After the vaccination, I experienced soreness, fever, and redness and swelling in the injection site and underarm lymph nodes.
About a week later, the discomfort subsided, but I noticed a bruise at the injection site due to the redness and swelling.
Since it no longer hurt, I didn't pay much attention to it.
Recently, I discovered that the area where I was injected is still slightly swollen and hard, and I can feel it when I press on it, but it is not very painful.
I would like to know what could be causing this situation.
Should I see a doctor for further examination, or would heat compress and massage be sufficient to resolve it?

Hello: The redness, swelling, and pain in your arm initially may be a "local injection site reaction" following the administration of the Moderna vaccine.
This typically resolves gradually within a week.
Based on your description, although the discomfort in your arm has improved, there is still a lump and tenderness present, and since it has been over a month since March 10, it is advisable for you to seek medical evaluation and management for peace of mind.

More Info

After receiving a vaccine, it is not uncommon to experience some localized side effects at the injection site, including swelling, redness, and tenderness. These reactions are generally part of the body's immune response to the vaccine, as it works to build immunity against the targeted disease. However, the persistence of swelling or the development of a hard lump at the injection site, as you described, can raise concerns and warrants further discussion.

Understanding Post-Vaccine Reactions
1. Normal Reactions: After vaccination, it is typical to experience soreness, swelling, and redness at the injection site. These symptoms usually resolve within a few days to a week. The swelling is often due to inflammation as the immune system responds to the vaccine.
2. Lymphadenopathy: The swelling of lymph nodes, particularly in the armpit area, can occur as the body processes the vaccine. This is a sign that your immune system is actively responding. However, if the swelling persists beyond a week or becomes painful, it may require further evaluation.

3. Hematoma or Localized Reaction: The hard lump you are feeling could be a hematoma (a localized collection of blood outside of blood vessels) or a localized inflammatory reaction. These can occur if the vaccine was injected too deeply or if there was minor bleeding under the skin. While they often resolve on their own, they can take time to fully dissipate.

4. Infection: Although rare, there is a possibility of infection at the injection site. Signs of infection include increased redness, warmth, swelling, and pain, along with systemic symptoms like fever. If you notice any of these signs, it is important to seek medical attention.

When to Seek Medical Help
Given your situation, here are some guidelines on when to seek medical attention:
- Persistent Symptoms: If the swelling or hardness at the injection site does not improve over the next few days or worsens, it would be prudent to consult a healthcare provider.

- Pain or Discomfort: If you experience increasing pain or discomfort at the injection site, this could indicate a more serious issue that requires evaluation.

- Signs of Infection: If you notice any signs of infection (increased redness, warmth, fever, or pus), you should seek medical care immediately.

- Impact on Daily Life: If the symptoms are affecting your daily activities or causing significant concern, it is always better to err on the side of caution and consult a healthcare professional.

Self-Care Measures
In the meantime, you can try some self-care measures to alleviate discomfort:
- Warm Compress: Applying a warm compress to the area may help reduce swelling and promote blood flow, which can aid in healing.

- Gentle Massage: If it feels comfortable, gently massaging the area may help to break down any localized swelling.

- Over-the-Counter Pain Relief: If you are experiencing discomfort, over-the-counter pain relievers such as ibuprofen or acetaminophen can help manage pain and inflammation.

Conclusion
While localized swelling and tenderness after a vaccine are common and usually resolve on their own, persistent symptoms warrant further evaluation. It is essential to monitor your symptoms and seek medical advice if you have any concerns. Your health and peace of mind are important, and healthcare providers are there to help you navigate any post-vaccine reactions you may experience.
