Pain beneath the rib cage?
Hello Doctor, since around November of last year, I have been experiencing intermittent pain near the lower edge of my left rib cage close to the abdomen.
Currently, the frequency is about four times a week, and the pain lasts only a few seconds each time.
Last year, I thought it was stomach pain and consulted a gastroenterologist.
The doctor mentioned that this area is more related to the thoracic cavity and prescribed medication for acute gastroenteritis, but it was ineffective.
In addition to the pain, I have noticed that the lower edge of my left rib cage protrudes slightly more than the right side.
When I touch the painful area, it feels numb, but pressing on it does not cause pain.
Since it does not significantly affect my daily life, I haven't paid much attention to it, but I would still like to know what might be causing the pain.
I have anemia and asthma, and I regularly take iron-containing multivitamins and B-complex vitamins.
Thank you, Doctor.

Understanding pain under the left rib can be quite complex, as it may stem from a variety of causes, ranging from musculoskeletal issues to internal organ problems. Given your description of the pain occurring intermittently since November, along with the observation that the left rib area is more prominent than the right, several potential explanations can be considered.
1. Musculoskeletal Issues: One common cause of pain in the rib area is related to the muscles, ligaments, or bones. Conditions such as costochondritis, which is inflammation of the cartilage connecting the ribs to the sternum, can lead to localized pain. Additionally, muscle strain or injury, particularly from activities that involve twisting or heavy lifting, could also manifest as pain in this area. The fact that you feel a tingling sensation (described as "麻麻的") when touching the area could suggest nerve involvement, possibly due to muscle tension or irritation.
2. Gastrointestinal Causes: Although you mentioned that a gastroenterologist prescribed medication for acute gastroenteritis without relief, it’s still worth considering gastrointestinal issues. Conditions such as gastritis, peptic ulcers, or even issues related to the spleen could cause referred pain to the left rib area. The spleen, located in the upper left abdomen, can become enlarged or inflamed, leading to discomfort that may be perceived as rib pain.
3. Cardiac Considerations: While less likely given your age and the nature of the pain, it’s important to rule out any cardiac issues, especially if you have risk factors such as anemia and asthma. Sometimes, heart-related pain can be referred to the left side of the chest and rib area. If you experience any additional symptoms such as shortness of breath, dizziness, or radiating pain, it would be prudent to seek immediate medical attention.
4. Pulmonary Issues: Given your history of asthma, it’s also possible that respiratory issues could be contributing to your discomfort. Conditions such as pleuritis (inflammation of the lining of the lungs) or even pneumonia can cause pain that feels like it’s coming from the ribs. If you notice any changes in your breathing patterns or increased wheezing, this should be evaluated.
5. Anemia and Nutritional Deficiencies: You mentioned having anemia and taking iron supplements. Anemia can sometimes lead to generalized weakness and discomfort, but it’s less likely to be the direct cause of localized rib pain. However, nutritional deficiencies, particularly in B vitamins, can lead to nerve issues that might manifest as tingling or discomfort in various body areas.
Given that the pain is not significantly impacting your daily life, it may be tempting to ignore it. However, since it has persisted for several months, it would be advisable to follow up with a healthcare provider, particularly if the pain changes in intensity or frequency, or if new symptoms arise. A thorough physical examination, possibly including imaging studies such as an ultrasound or CT scan, may be warranted to rule out any underlying conditions.
In conclusion, while there are several potential causes for the pain under your left rib, a careful evaluation by a healthcare professional is essential for accurate diagnosis and appropriate management. Keeping track of any additional symptoms or changes in your condition will be helpful for your healthcare provider in determining the best course of action.
