White spots around the anal area?


Hello Dr.
Chang,
It seems like I have keratosis pilaris; the surface of the skin appears red and resembles papules.
When I squeeze it, a white, elongated substance comes out, which does not look like pus but more like comedones.
There are many spots around the anal area.
It hurts when I sit, and this has been going on for about six months.
The medications I have applied, including Le Yu hemorrhoid cream and other hemorrhoid ointments, as well as zinc oxide, have not improved the condition.
I have even tried applying Meikang cream.
Should I use an antibiotic ointment instead? A few months ago, I had an anal fissure, which I suspect was caused by taking aspirin.
After consulting my doctor, I was advised to temporarily stop taking aspirin, and this week the anal fissure has improved.
However, the white spot issue persists, and I still experience pain around the anal area.
