White bumps near the anus?

Hello doctor, recently I have developed a few off-white bumps near my anus.
The area around the bumps is red, and while they don't itch, they are painful when pressed.
What could this condition be? I have some eczema medication on hand; would that be effective, or should I go to the hospital for a check-up?

CHEN, 20~29 year old female. Ask Date: 2015/12/28

Dr. Li Yufen reply Dermatology

Hello: The most common possibilities are bacterial, fungal, or Candida infections.
The use of steroid ointments for eczema may worsen the condition.
It is recommended that you consult a nearby dermatologist for a definitive diagnosis before starting treatment.
Thank you!

White bumps near the anus can be concerning, and it’s important to understand the potential causes and treatment options available. Based on your description of the symptoms—white bumps that are painful when pressed but not itchy, and surrounded by redness—there are several possibilities to consider.

One common cause of white bumps in the anal area is pilonidal cysts. These are often found near the tailbone and can become inflamed or infected, leading to painful bumps. Another possibility is sebaceous cysts, which are non-cancerous lumps that can occur when a hair follicle or oil gland becomes blocked. These cysts can also become inflamed, resulting in pain and redness.

Another potential cause could be hemorrhoids, which can sometimes present as lumps near the anus. While hemorrhoids are typically associated with itching and bleeding, they can also cause discomfort and pain, especially if they become thrombosed (clotted).
Additionally, anal warts, caused by the human papillomavirus (HPV), can appear as small, white bumps around the anal area. These may not always be painful but can cause discomfort depending on their size and location.

Given that you have eczema medication on hand, it’s important to note that while topical treatments for eczema can help with inflammation and irritation, they may not be effective for cysts or warts. Using eczema cream on a condition that is not eczema could potentially worsen the situation or delay proper treatment.

In terms of treatment options, it is crucial to first obtain an accurate diagnosis from a healthcare professional. Here are some general steps you can take:
1. Consult a Healthcare Provider: It is advisable to see a dermatologist or a healthcare provider who specializes in skin conditions. They can perform a physical examination and may recommend further tests if necessary.

2. Avoid Self-Treatment: While it may be tempting to use over-the-counter treatments or home remedies, it is best to avoid self-treatment until you have a clear diagnosis. This is particularly important if the bumps are painful or inflamed.

3. Follow Medical Advice: If diagnosed with a specific condition, follow the treatment plan provided by your healthcare provider. This may include topical treatments, oral medications, or in some cases, surgical intervention if cysts or warts are present.

4. Maintain Hygiene: Keeping the area clean and dry can help prevent further irritation or infection. Avoid using harsh soaps or irritants that could exacerbate the condition.

5. Monitor Symptoms: Keep track of any changes in the bumps, such as increased pain, changes in size, or the development of new symptoms. This information can be helpful for your healthcare provider.

In summary, while there are several potential causes for the white bumps near your anus, it is essential to seek professional medical advice for an accurate diagnosis and appropriate treatment. Self-treating with eczema medication may not be effective and could lead to complications. Prioritize your health by consulting a healthcare provider to address your concerns effectively.
