Regarding the issue of binocular diplopia?
Hello Doctor: In the past month or two, I have been experiencing double vision in certain situations.
I have seen an ophthalmologist, but no abnormalities were found.
The problem initially arose when I was focusing straight ahead at work; the two rods I was looking at could not align in a straight line.
When I focused on one rod, the other appeared doubled.
Currently, when I test my eyes using two pens held upright in front of me, if I focus on Pen A, I see double vision with Pen B, and vice versa.
I have tried various things; it seems similar to holding a broomstick, where I experience the same double vision when focusing on objects in front of me.
However, if the objects are thicker or farther away, I do not experience this issue.
In my daily life, I notice this particularly when looking at railings at home or distant objects, and I also encounter it during work-related tasks.
I would like to ask the doctor how I can resolve this situation.

Hello: I cannot determine your condition, but based on your description, focusing on pen A while looking at pen B causes double vision, and vice versa.
This is a completely normal phenomenon; everyone experiences this, so there is no need for concern.
Here are some possible causes related to the symptoms you described:
1.
Refractive issues: Refractive errors can lead to double vision.
This may include myopia, hyperopia, astigmatism, or asymmetry in the refractive power of the eyes.
An ophthalmologist can test your vision to determine your refractive status and make appropriate adjustments to your glasses or contact lenses.
2.
Ocular muscle issues: Double vision may be related to the coordination of the eye muscles.
This could be due to muscle weakness, ocular motility disorders, or other muscular problems.
Further evaluation may require vision tests, eye movement assessments, and other possible examinations.
3.
Eye fatigue and visual habits: Prolonged focus on near tasks, extended use of electronic devices, or poor visual habits may lead to eye fatigue and visual discomfort.
Try to follow good visual habits, including taking breaks from screen time, resting your eyes periodically, and using artificial tears.

More Info
Double vision, or diplopia, can be a perplexing and distressing condition that significantly impacts daily life. It can manifest in various ways, such as seeing two images of a single object, which may be horizontal, vertical, or diagonal. Understanding the underlying causes and potential solutions is crucial for effective management.
Understanding Double Vision
Double vision can arise from several factors, including:
1. Refractive Errors: Issues with how the eye focuses light can lead to diplopia. Conditions like astigmatism or uncorrected vision problems can cause images to appear doubled.
2. Muscle Imbalance: The eyes are controlled by six muscles that must work in harmony. If one muscle is weaker or stronger than its counterpart, it can lead to misalignment and double vision. This is often seen in conditions like strabismus (crossed eyes).
3. Neurological Issues: Conditions affecting the brain or nerves that control eye movement can lead to diplopia. This includes strokes, multiple sclerosis, or myasthenia gravis.
4. Cataracts: As cataracts develop, they can cause light to scatter, leading to double vision, particularly in one eye.
5. Other Medical Conditions: Diabetes, thyroid disorders, and high blood pressure can also contribute to double vision.
Steps to Address Double Vision
1. Comprehensive Eye Examination: Since you have already seen an eye doctor without finding abnormalities, it may be beneficial to seek a second opinion, preferably from a specialist in neuro-ophthalmology. They can conduct a more thorough examination, including tests for eye muscle function and neurological assessments.
2. Vision Correction: If refractive errors are identified, corrective lenses may help alleviate symptoms. In some cases, prism glasses can be prescribed to help align the images seen by each eye.
3. Eye Muscle Surgery: If the double vision is due to muscle imbalance, surgical options may be available to realign the eyes. This is typically considered when other treatments have not been effective.
4. Neurological Evaluation: If there is a suspicion of a neurological cause, a referral to a neurologist may be necessary for further evaluation, including imaging studies like an MRI or CT scan.
5. Therapeutic Approaches: Vision therapy may be recommended to strengthen the eye muscles and improve coordination. This is often a gradual process and requires commitment.
6. Lifestyle Adjustments: In the interim, you might consider strategies to cope with double vision, such as closing one eye during tasks that require focus, using adequate lighting, and avoiding situations that exacerbate the symptoms.
Conclusion
Double vision can be a complex issue with various underlying causes. It is essential to approach this condition comprehensively, considering both ocular and neurological factors. If you continue to experience symptoms, do not hesitate to seek further evaluation from specialists. Remember, managing double vision often requires a multi-faceted approach, and persistence in seeking the right care is key to finding relief.
