Pain in the front side and underneath of the big toe?
Doctor, my big toe is experiencing pain around the nail and underneath the toe.
It feels sore and has a tingling sensation.
There is slight redness on the surface, and taking medication for gout has not been effective.
I would like to ask the doctor what this could possibly be.
Thank you.

Dr. Wu Mukhan reply Neurology
Hello, gout typically occurs at the metatarsophalangeal joint of the big toe, which is different from the location you described and is less likely to be gout.
If there is redness, swelling, and pain, there may be inflammation or infection.
It is recommended to visit a dermatologist or a general surgery clinic for further evaluation.

More Info
The pain you are experiencing around your big toe, particularly near the nail and the underside of the toe, can be attributed to several potential causes. Given the symptoms of soreness, tingling, and slight redness, it is essential to consider a few common conditions that could be responsible for your discomfort.
1. Gout: Although you mentioned that you are taking medication for gout and it has not been effective, it is still worth considering. Gout is a type of inflammatory arthritis caused by the accumulation of uric acid crystals in the joints, often affecting the big toe. Symptoms typically include intense pain, swelling, and redness. If your current medication is not alleviating the symptoms, it may be necessary to consult your physician for a reevaluation of your treatment plan or to explore alternative medications.
2. Ingrown Toenail: This condition occurs when the edge of the toenail grows into the surrounding skin, leading to pain, redness, and swelling. If you notice that the pain is localized around the nail area and there is visible redness, this could be a likely cause. Treatment often involves proper nail trimming techniques, soaking the foot in warm water, and in some cases, surgical intervention may be necessary to remove the ingrown portion of the nail.
3. Bunion: A bunion is a bony bump that forms at the base of the big toe, often causing the toe to angle towards the second toe. This condition can lead to pain and discomfort, especially when wearing tight shoes. If you have a noticeable bump on the side of your big toe, this could be the issue. Treatment options include wearing wider shoes, using orthotic devices, and in severe cases, surgery may be required.
4. Neuroma: A neuroma is a thickening of nerve tissue that can occur between the toes, often leading to sharp, burning pain. While this typically affects the area between the third and fourth toes, it can sometimes cause discomfort in the big toe area as well. Treatment may involve rest, ice, anti-inflammatory medications, or corticosteroid injections.
5. Metatarsalgia: This condition refers to pain and inflammation in the ball of the foot, which can radiate to the toes. It can be caused by various factors, including improper footwear, high-impact activities, or foot deformities. Treatment usually involves rest, ice, and supportive footwear.
6. Infection: If there is significant redness, swelling, and warmth around the toe, it could indicate an infection, such as cellulitis or an infected ingrown toenail. In such cases, it is crucial to seek medical attention promptly, as infections may require antibiotics or other interventions.
Given the variety of potential causes for your symptoms, it is advisable to consult a healthcare professional for a thorough examination. They may recommend imaging studies, such as X-rays, to rule out any structural issues or other diagnostic tests to determine the underlying cause of your pain.
In the meantime, you can try some home care measures, such as soaking your foot in warm water with Epsom salt, wearing comfortable and supportive footwear, and avoiding activities that exacerbate the pain. Over-the-counter anti-inflammatory medications may also help alleviate discomfort, but be sure to consult with your healthcare provider before starting any new medication, especially if you have existing health conditions or are taking other medications.
In summary, while there are several potential causes for the pain around your big toe, a thorough evaluation by a healthcare professional is essential to determine the exact cause and appropriate treatment plan.
