Painful sensation at the tip of the big toe?

I have been experiencing a sharp pain at the tips of both my big toes recently.
However, I couldn't find any similar symptoms online.
The pain is located just below the front of the toenail and is in an area that does not touch the ground when standing.
Normally, there is no reaction, but any pressure on the toe causes pain, even when a thin blanket slides over the toes.
Could you tell me what this symptom might be and which specialist I should see?

Hello, it seems that your symptoms may be related to peripheral nerve issues.
If there is no improvement, it is recommended to visit a neurology outpatient clinic for evaluation to determine if further testing is necessary.

The symptoms you are experiencing in the tips of your big toes, characterized by a sharp pain or tingling sensation, can be attributed to several potential causes. Given that the pain occurs specifically when pressure is applied to the area, it suggests that there may be an underlying issue related to the nerves, blood flow, or structural integrity of the toes.

One possible cause of your symptoms could be neuropathy, which is a condition that affects the peripheral nerves. Neuropathy can lead to sensations of tingling, burning, or sharp pain, particularly in the extremities. This condition can be caused by various factors, including diabetes, vitamin deficiencies (especially B vitamins), or even alcohol use. If you have any underlying health conditions, it would be wise to consider these as potential contributors.

Another possibility is metatarsalgia, which refers to pain and inflammation in the ball of the foot. While this typically affects the area just behind the toes, it can sometimes lead to referred pain or discomfort in the toes themselves. This condition can be exacerbated by wearing ill-fitting shoes or engaging in activities that put excessive pressure on the forefoot.

You might also be experiencing Morton's neuroma, which is a thickening of the tissue around one of the nerves leading to your toes. This condition often causes sharp, burning pain in the ball of the foot and can radiate to the toes. It is commonly associated with wearing tight shoes or high heels, which compress the toes.

Additionally, fungal infections or ingrown toenails can cause localized pain in the toe area. If there is any visible swelling, redness, or discharge, these could be signs of an infection or an ingrown nail that may require medical attention.

Given the specific nature of your symptoms, it is advisable to consult with a podiatrist, a specialist in foot and ankle disorders. They can perform a thorough examination, possibly including imaging studies like X-rays or MRIs, to determine the exact cause of your pain. They may also assess your footwear and gait to identify any contributing factors.

In the meantime, here are some general recommendations to manage your symptoms:
1. Footwear: Ensure that you are wearing well-fitting shoes that provide adequate support. Avoid high heels or shoes that compress the toes.

2. Rest: If you notice that certain activities exacerbate your pain, try to limit those activities until you can see a specialist.

3. Ice: Applying ice to the affected area can help reduce inflammation and numb the pain.

4. Over-the-counter pain relief: Non-steroidal anti-inflammatory drugs (NSAIDs) like ibuprofen can help alleviate pain and reduce inflammation.

5. Foot exercises: Gentle stretching and strengthening exercises for your feet may help improve flexibility and reduce discomfort.

6. Avoid pressure: Try to avoid putting pressure on the affected area, especially when sitting or lying down.

In conclusion, while your symptoms may seem isolated, they could be indicative of a broader issue that requires professional evaluation. A podiatrist will be able to provide a more accurate diagnosis and recommend appropriate treatment options tailored to your specific condition.
