Medical consultation (upon waking up in the morning)
Hello Doctor, I am Xiao Wu.
My height is 179 cm and my weight is 93 kg.
On the morning of June 7th, I woke up at 6 AM and began to feel pain in the liver area (mild pain with breathing and more intense pain when pressed).
However, if I avoid deep breathing or intentional pressure, I only experience pain when breathing.
Then, on the morning of June 8th, I woke up at the same time and noticed another area of pain, which is in the upper back (likely the ribs), with the same issue.
I would like to ask if these two areas need X-rays or other examinations, or could it simply be due to pressure from my sleeping position? I always sleep in a proper position, so I really don’t understand why the pain started so suddenly.
Regarding my routine: I don’t like taking leave from work, so since I started working at 15, I have rarely gone to the hospital.
Whenever I have an issue, I usually take over-the-counter medication on weekends or just wait for it to heal naturally without seeing a doctor.
In the past three years, I have experienced three episodes of back strain, which I mostly treated with adhesive patches and waited for them to heal naturally.

Morning pain in the liver and rib area can be concerning, especially when it occurs suddenly and is accompanied by discomfort during breathing or pressure. Given your description, it is advisable to seek medical attention for a thorough evaluation.
The liver is located in the upper right quadrant of the abdomen, and pain in this area can sometimes be related to liver conditions, but it can also stem from other sources such as the gallbladder, ribs, or even musculoskeletal issues. The fact that you experience pain upon pressing the area and during deep breaths suggests that it might not solely be a liver issue. It could be related to muscle strain, rib dysfunction, or even referred pain from other organs.
Your history of occasional back pain and your tendency to self-medicate without seeking professional help could be contributing factors to your current symptoms. Chronic pain or discomfort that persists or worsens over time should not be ignored. It’s essential to differentiate between musculoskeletal pain and pain that may indicate an underlying organ issue.
In your case, the sudden onset of pain after waking up could be due to several reasons:
1. Muscle Strain or Sprain: If you've had an unusual sleeping position or if you've engaged in activities that may have strained your back or abdominal muscles, this could lead to localized pain.
2. Rib Dysfunction: Sometimes, the ribs can become misaligned or strained, leading to pain that can be exacerbated by deep breathing or movement.
3. Gastrointestinal Issues: Conditions such as gastritis, gallbladder disease, or even acid reflux can cause discomfort in the upper abdomen and may mimic liver pain.
4. Liver Conditions: Given your history as a hepatitis B carrier, it is crucial to monitor liver health closely. While your symptoms may not directly indicate liver issues, any new or unusual pain should prompt a check-up, especially considering the potential for complications related to chronic liver disease.
5. Referred Pain: Sometimes, pain in the liver area can be referred from other areas, such as the spine or kidneys.
Considering these possibilities, it is recommended that you undergo diagnostic imaging, such as an ultrasound or X-ray, to rule out any serious conditions. Blood tests to assess liver function and check for any signs of inflammation or infection may also be warranted.
In summary, while it is possible that your symptoms could be related to something as simple as a muscle strain or poor sleeping posture, the presence of pain in the liver and rib area, especially given your medical history, necessitates a professional evaluation. Early diagnosis and treatment can prevent potential complications and ensure that any underlying issues are addressed promptly. Don't hesitate to seek medical advice; your health is paramount.
