Should Wisdom Teeth Be Removed Before Orthodontic Treatment? - Dentistry

Do I need to extract my wisdom teeth before undergoing orthodontic treatment?

Hello Doctor, I currently have all four wisdom teeth erupted.
The upper left and right wisdom teeth are growing normally in a vertical position, while the lower left wisdom tooth is also vertical but has surrounding gum tissue that has not fully erupted.
The lower right wisdom tooth is horizontal, with only a portion of it visible and a gap between it and the adjacent tooth.
During my last military health examination, the doctor diagnosed that the upper wisdom teeth do not have issues with impaction or treatability, but all lower wisdom teeth were diagnosed as impacted, with the right side being classified as treatable or untreatable.
I have not had a dental X-ray taken recently, so I am unaware of the current situation, but I will arrange for a dental cleaning and examination soon.
During the cleaning, I will use interdental brushes to clean the gaps around the wisdom teeth, and sometimes the lower right wisdom tooth bleeds.
I would like to ask the doctor whether it is advisable to extract the lower wisdom teeth before proceeding with orthodontic evaluation, or should I first undergo the orthodontic evaluation and then let the doctor determine if the lower wisdom teeth need to be extracted? Thank you, Doctor.

Hello: It is recommended to first undergo an orthodontic evaluation and confirm with the orthodontist whether there is sufficient space in the dental arch, among other concerns.
If you are extremely reluctant to extract teeth, it is advisable to discuss this thoroughly with the orthodontist, including the potential compromises in space and whether the final results will meet your expectations if extractions are avoided.

When considering orthodontic treatment, the management of wisdom teeth (third molars) is a common concern. Your situation, with varying conditions of your wisdom teeth, requires careful evaluation to determine the best course of action.
Understanding Wisdom Teeth and Orthodontics
Wisdom teeth can often become problematic due to their positioning and the limited space in the jaw. In your case, you have two fully erupted wisdom teeth in the upper jaw and two in the lower jaw, with one being horizontally impacted. The condition of these teeth can significantly influence your orthodontic treatment plan.

1. Impacted Wisdom Teeth: The lower right wisdom tooth, which is horizontally impacted, poses a risk of crowding and can affect the alignment of adjacent teeth. If left untreated, it may lead to complications such as infection, pain, or damage to neighboring teeth. Orthodontists often recommend the removal of impacted wisdom teeth before starting braces to prevent these issues.

2. Erupted Wisdom Teeth: The upper wisdom teeth appear to be in a better position, but they can still cause problems if they exert pressure on the adjacent molars. If they are not causing any immediate issues, they may not need to be removed before orthodontic treatment. However, if there is any sign of crowding or if they are contributing to misalignment, extraction may be considered.

3. Orthodontic Evaluation: It is generally advisable to have a comprehensive orthodontic evaluation before making a decision about wisdom teeth extraction. This evaluation typically includes X-rays to assess the position of the wisdom teeth and their relationship with the rest of your dental structure. The orthodontist will consider factors such as the alignment of your teeth, the potential for crowding, and the overall health of your gums and teeth.

4. Timing of Extraction: If the orthodontist determines that the lower wisdom teeth should be removed, it is often best to do this before starting braces. This allows for optimal space management and alignment during the orthodontic process. However, if the wisdom teeth are not causing any issues, the orthodontist may recommend monitoring them throughout treatment.

5. Post-Extraction Considerations: If extraction is necessary, it’s important to allow adequate healing time before beginning orthodontic treatment. This typically involves a few weeks of recovery, during which the extraction sites heal and any swelling subsides.

6. Oral Hygiene: Given that you have experienced bleeding while cleaning around your wisdom teeth, it’s crucial to maintain excellent oral hygiene. This includes regular brushing, flossing, and using interdental brushes to keep the area clean. If you notice persistent bleeding or discomfort, it’s advisable to consult your dentist or oral surgeon for further evaluation.

Conclusion
In summary, the decision to remove wisdom teeth before orthodontic treatment should be based on a thorough evaluation by an orthodontist. They will assess the positioning of your wisdom teeth, their potential impact on your orthodontic goals, and your overall dental health. If extraction is recommended, it should ideally be done before starting braces to ensure the best possible outcome for your treatment. Always communicate openly with your dental care providers about your concerns and preferences, as they can guide you through the process and help alleviate any anxieties you may have regarding the treatment.
