Fell and hit the back of the head?
Hello, doctor.
Yesterday, I accidentally fell and hit the back of my head.
It immediately swelled up significantly, and aside from the redness, swelling, and pain at the impact site, there were no other unusual symptoms that day.
I applied ice, and today the swelling has decreased a lot, but it still hurts a bit when I touch it, and the scalp is still red.
Today, my neck feels a bit sore as well, but there are no other abnormal symptoms.
Does this mean there shouldn't be any major issues? What else should I pay attention to? Is it necessary to see a doctor? Should I continue applying ice? Thank you.

After experiencing a fall that results in a head injury, it is crucial to monitor your symptoms closely, as head injuries can sometimes lead to serious complications. Based on your description, it seems that you have sustained a contusion (bruise) on your scalp, which is characterized by swelling, redness, and localized pain. The fact that you applied ice and noticed a reduction in swelling is a positive sign, as ice can help to minimize inflammation and alleviate pain in the initial stages following an injury.
However, it is essential to remain vigilant for any changes in your condition. While the absence of severe symptoms such as loss of consciousness, persistent vomiting, severe headache, confusion, or weakness is reassuring, you should still be aware of the potential for delayed symptoms. For instance, headaches, dizziness, or changes in vision can sometimes develop hours or even days after the injury.
The neck pain you are experiencing could be related to muscle strain from the fall or tension from bracing yourself during the impact. It is not uncommon for individuals to experience neck discomfort following a head injury, especially if there was a sudden movement or jolt. Gentle stretching, heat application, and over-the-counter pain relief may help alleviate this discomfort.
In terms of ongoing care, here are some recommendations:
1. Rest: Ensure you get plenty of rest. Your body needs time to heal, and adequate sleep is crucial for recovery.
2. Ice Application: Continue to apply ice to the affected area for the first 24 to 48 hours post-injury to reduce swelling. After this period, you may switch to heat therapy to help with muscle relaxation and pain relief.
3. Monitor Symptoms: Keep a close eye on your symptoms. If you notice any new symptoms such as severe headaches, confusion, difficulty concentrating, or any neurological changes (like weakness or numbness), seek medical attention immediately.
4. Limit Strenuous Activities: Avoid activities that could risk another fall or injury, including sports or heavy lifting, until you feel fully recovered.
5. Hydration and Nutrition: Stay hydrated and maintain a balanced diet to support your recovery.
6. Follow-Up: If your symptoms persist or worsen, it is advisable to consult a healthcare professional. Even if you feel that your injury is minor, a medical evaluation can rule out any underlying issues such as a concussion or intracranial bleeding.
In summary, while your current symptoms may not indicate a severe injury, it is essential to remain cautious and monitor your condition closely. If you have any doubts or if your symptoms change, do not hesitate to seek medical advice. Remember, it is always better to err on the side of caution when it comes to head injuries.
