Blackening of genital acne after squeezing?

Hello doctor, about two weeks ago, I noticed a bump that seems to be a pimple just above the base of my penis (not quite at the pubic hair area).
It was itchy, but I didn't pay much attention to it since I had experienced something similar before.
I applied Neomycin ointment on my own, but later I noticed it developed a scab and turned black.
Then, a little higher up, another bump appeared.
A few days later, I accidentally squeezed it a bit too hard, and it also formed a scab and turned black, and it was itchy as well.
I consulted two doctors, but they both said they didn't know what it was and only prescribed Clotrimazole cream.
I've been using it for almost a week, but the black color hasn't faded, although no new bumps have appeared.
Currently, the condition is sometimes itchy and sometimes not.
What type of bump could this be?

Hello: If it is skin hyperpigmentation, it usually only lightens and does not completely disappear.
If it is a scab, it may become slightly red after falling off over time, and then gradually return to normal.
If you are concerned, you can apply an ointment for a while.
Take good care of your skin, and if it does not improve, you can seek medical attention at a hospital.
Wishing you good health.

When it comes to the appearance of blackened pimples on the penis, there are several potential causes and treatment options to consider. Based on your description, it seems that you have developed a couple of lesions that initially resembled pimples but have since become crusted and blackened. This can be concerning, especially when accompanied by itching and discomfort.

Firstly, it’s important to understand that the skin in the genital area is sensitive and can react to various factors, including irritation, infection, or even friction. The initial pimple-like lesions you described could have been folliculitis, which is an inflammation of the hair follicles often caused by bacteria, fungi, or irritation. When these lesions become crusted and black, it may indicate that they have scabbed over due to either healing or secondary infection.

The use of Neomycin ointment, which is an antibiotic, may not have been effective if the underlying cause was not bacterial. The fact that you have seen two doctors who were unable to provide a definitive diagnosis can be frustrating. It’s possible that the lesions could be related to a condition such as a viral infection (like herpes), a fungal infection, or even a benign skin condition like seborrheic keratosis or a wart.

The fact that the lesions are itchy and have not improved with topical treatments suggests that further evaluation may be necessary. It’s crucial to avoid picking at or squeezing the lesions, as this can lead to further irritation, infection, or scarring.
In terms of treatment, since you have been prescribed clotrimazole cream (which is an antifungal), it’s essential to continue using it as directed. If there is no improvement after a week or so, or if the lesions worsen, it would be advisable to return to a dermatologist for a more thorough examination. A skin biopsy may be warranted to rule out other conditions, especially if the lesions persist or change in appearance.

Additionally, maintaining good hygiene in the genital area is crucial. Ensure that you are washing gently with mild soap and water, and avoid using harsh products that could irritate the skin. Wearing loose-fitting underwear and clothing can also help reduce friction and irritation.

In summary, while the blackened pimples on your penis could be due to a variety of causes, including folliculitis or other skin conditions, it is essential to seek further medical evaluation if the symptoms persist. A dermatologist can provide a more accurate diagnosis and recommend appropriate treatment options based on a physical examination and possibly further testing. Remember, it’s always better to err on the side of caution when it comes to changes in your skin, especially in sensitive areas.
