Redness and swelling of the inner foreskin?

Hello doctor, I started experiencing some redness and itching on the inner side of my foreskin two days ago, but yesterday it began to swell.
I had sexual intercourse a week ago, and I used a condom, so I believe it shouldn't be related to that.
I also bought Clotrimazole cream yesterday to apply, as I have no previous history of related issues.
I would like to ask if using Clotrimazole cream is appropriate? How long might the treatment take? Is there a need to go to the hospital for an examination?

Niming, 40~49 year old female. Ask Date: 2023/06/28

Dr. Du Shixi reply Urology

Hello: Keep the area clean and dry.
If there is no improvement after using the topical ointment for 1-2 days, please seek medical attention.
Generally, it should improve within 3-5 days.
Wishing you good health.

More Info

Redness and swelling inside the foreskin can be concerning, and it's important to understand the potential causes and appropriate treatments. Based on your description, it seems you are experiencing symptoms that could be related to several factors, including irritation, infection, or an allergic reaction.

Possible Causes
1. Irritation: This can occur due to friction during sexual activity, especially if there was insufficient lubrication. Even with a condom, friction can lead to irritation of the sensitive skin inside the foreskin.

2. Infection: Conditions such as balanitis (inflammation of the glans) or posthitis (inflammation of the foreskin) can cause redness and swelling. These infections can be caused by bacteria, yeast (like Candida), or sexually transmitted infections (STIs). Since you mentioned having sexual intercourse a week ago, it is possible that an infection could have developed.

3. Allergic Reaction: Sometimes, reactions to latex condoms or lubricants can cause localized redness and swelling. If you have recently changed brands or types of condoms, this could be a factor.

Treatment Options
Using a topical antifungal cream like Clotrimazole (the active ingredient in "卡黴速停") can be appropriate if you suspect a yeast infection, especially if you notice additional symptoms like itching or a discharge that resembles cottage cheese. However, if the redness and swelling persist or worsen, it is crucial to seek medical attention.

When to See a Doctor
1. Persistent Symptoms: If the redness and swelling do not improve within a few days of using the antifungal cream, or if they worsen, you should see a healthcare provider.

2. Severe Symptoms: If you experience severe pain, fever, or any discharge that is unusual (especially if it is foul-smelling), you should seek medical attention immediately.

3. Recurrent Issues: If you have had similar symptoms in the past or if this is not your first experience with such issues, it is advisable to consult a doctor for a thorough examination.

Duration of Treatment
The duration of treatment can vary depending on the underlying cause. If it is a simple case of irritation, symptoms may resolve within a few days with proper care. However, if it is an infection, it may take longer, and you may need a prescription for oral medications or stronger topical treatments.

Conclusion
In summary, while using Clotrimazole may help if you suspect a yeast infection, it is essential to monitor your symptoms closely. If there is no improvement or if symptoms worsen, do not hesitate to seek medical advice. A healthcare provider can perform a physical examination and may recommend tests to determine the exact cause of your symptoms, ensuring you receive the appropriate treatment. Remember, early intervention can prevent complications and promote faster healing.
